What are we trying to accomplish? • MEASUREMENT • Monthly % of teams submitting How will we know that a change is an improvement? • CYCLE FOR LEARNING AND IMPROVEMENT • Establish contact with designated teams • Act as a resource and help them understand how to collect and submit the data • Coach teams through the change process • Initiate contacts with enrolled organizations in your LHIN • Focus on the quality improvement issues What changes can we make that will result in improvement?
