  2. PICO? • P: patienten met verdenking op CVST (of CVST in DD) • I: D-dimeren bepaling • C: CVST uitgesloten • O: Een CVST d.m.v. D-dimeren uitgesloten

  3. Waarom D-dimeren? • Er zijn veel mensen met hoofdpijn en met CVST in de DD. • MRA-MRV is tijdrovend, vraagt veel personele inspanning en is kostbaar. • Een snelle goedkope test ter uitsluiting van een CVST zou MRA-MRV overbodig maken.

Dus: • Eigenlijk maar een goed onderzoek (Kosinski, Stroke dec 2004) • diagnose CVST erg onwaarschijnlijk indien: • D-dimeren normaal • Symptomen < 2 wk • Leeftijd  18 jr. • En geen zwangerschap en/of puerperium en/of abortus <4 wk, en/of maligniteit, en/of DVT/PE < 4 wk, en/of sepsis, en/of DIS, en/of acuut nefrotisch syndroom.

  17. Conclusie: • Een CVST is niet 100% uitgesloten bij negatieve D-dimeren. • CVST lijkt wel onwaarschijnlijk bij negatieve D-dimeren.
