Hillcrest Medical Center Tulsa, Oklahoma average daily census 390-400 patients. . 1998: Suicide of hospitalized patients recognized as a reportable sentinel event2007: TJC established a National Patient Safety Goal aimed at identifying patients at risk for suicide and identifying safety risks that might be found within the hospital2008: TJC recognized a record number of inpatient suicides2009: Patient suicide ranked as the second most reported sentinel event.
