Moderate Risk/Hazard When reporting a risk/hazard that you deem a moderate threat to occupational health & safety please utilise the standard electronic reporting form (Simwen Consulting Group Incident/Hazard Reporting Form) available on the Simwen Intranet.  Incident/Hazard Reporting Forms will be sent directly to the OHS Representative for evaluation. 21stof May 2012

  5. Minor Risk/Hazard Risks/hazards deemed of minor concern can be brought to the attention of the OHS Representative at the fortnightly general meeting (see Simwen Meeting Schedule for times).
