PROJECT DESCRIPTION • SWPF is a Savannah River Site capital asset project being designed, constructed, commissioned and operated for one year by Parsons and its teaming partners • Mission is to process high-Curie salt waste currently stored in underground tanks into three waste streams that are suitable for immobilization at downstream facilities: • Sludge containing actinides and Sr – vitrification and eventual transfer to the offsite national repository • Solution containing high curie Cs – vitrification and eventual transfer to the offsite national repository • Decontaminated salt solution - solidified with cementitious grout mixture • Received DOE approval of Performance Baseline (CD-2) and permission to start Limited Construction (CD-3A) in September 2007 • DOE approval for full construction targeted for August 2008

  4. CONSTRUCTION WORK CONTROL PROCESS Three levels of the Construction Work Control Process are: • Work Authorization • Control Account • Work Package

WORK AUTHORIZATION • All construction work contained in a Work Breakdown Structure (WBS) element must be authorized by the Project Manager before it can be performed • The Work Authorization Document (WAD) is the vehicle by which the Project Manager authorizes the work to be performed • It is an accountability agreement between the Project Manager and the Construction Manager to safely manage the work to the budget, schedule, and scope • It is Construction Manager responsibility to ensure sufficient budget and resources are allocated within the WAD to perform the work safely

CONTROL ACCOUNTS • Construction Manager assigns Control Account Manager (CAM) to develop the Control Account Plan (CAP) and manage its work scope • The CAP represents the construction work assigned within one WBS element and is normally discipline based • It is CAM responsibility to ensure sufficient budget and resources are allocated within the CAP to perform the work safely

WORK PACKAGES • The Work Package (WP) defines the lowest level of work scope within a CAP • Multiple WPs are required to execute total work scope in CAP • For self-performed work, CAM is responsible for WP development • CAM may use Subject Matter Experts and Line Worker Representatives to assist in WP development • WP is prepared to a level commensurate with and tailored to the hazards associated with the scope of work • Subcontract WPs are approved by Construction Manager and must comply with the minimum requirements for a self-performed WP

WORK PACKAGE CONTENTS • Detailed and bounding written description of the scope of work • Job Hazard Analysis (JHA) established for the scope of work (must be approved by Construction Manager) • Written work instructions • Necessary permits • Listing of interdiscipline and/or inter-area interfaces • Listing of special tools, equipment or training required • Listing of necessary hazard controls

WORK PACKAGE APPROVAL AND RELEASE • Resident Engineering Liaison reviews for concurrence of technical scope • Construction QC Manager reviews for concurrence on all quality related issues • Construction Safety Manager reviews for concurrence on JHA, hazard controls and assurance that Lessons Learned reviews were performed • Preparatory meeting and work site inspection is performed, WP revised as necessary • CAM signs for WP approval and authorization to begin work

WORK AUTHORIZATION • Daily Construction Coordination Meeting coordinates new work activities with on-going work activities (ensures worker safety, minimizes conflicts and down time) • General Superintendent authorizes new work activities (i.e., WPs released for field work) to be performed at this meeting ONLY after assurance that they can be performed safely • After release of a WP, authorization for field work to commence is given by the cognizant supervisor ONLY after Safe Work Plan (SWP) is developed and reviewed/signed-off by all workers

SAFE WORK PLAN • A task driven planning tool used to ensure that related tasks receive an evaluation of potential hazards prior to performing the work • The evaluation is performed at the job-specific level and involves the crew assigned to perform the work, with guidance from the responsible supervisor • SWP identifies: • Major steps of the work activity • Potential hazards of the work activity • A safe plan to deal with potential hazards

  12. SAFE WORK PLAN PREPARATION • Responsible supervisor conducts a walk through survey of the work area to identify hazards, need for permits, interdiscipline interferences, or other safety issues. Workers may accompany supervisor on walk through. • Supervisor, with assistance from workers, uses SWP Tools (JHA, Think Sheet, SWP Checklist, and Fall Proofing Checklist, etc.) to complete SWP. SWP must identify known hazards, applicable engineering and administrative controls, necessary permits, and required Personal Protective Equipment

  13. SAFE WORK PLAN USE • Supervisor conducts interactive briefing with entire crew. All crew members must sign SWP before work can commence • Interactive briefing must be repeated every day prior to start of shift • Interactive briefings must also be conducted: • After any change in worker personnel • When there is a change of task, hazards, or work conditions • When personnel not associated with the job require entry into the work area

  14. SAFE WORK PLAN USE • At the end of the work shift or as necessary, Supervisor reviews SWP for improvements or to address feedback provided by the work crew • Any time work scope changes, a deficiency is identified in the SWP, or changing conditions create new or potential hazards • Work must stop • SWP must be revised as necessary • General Superintendent must authorize resumption of work • Subcontractors must comply within minimum SWP requirements

  15. APPLICATION OF HUMAN PERFORMANCE IMPORVEMENT TO SWP PROCESS SWP form uses SAFER type questions to enhance error prevention • Are critical activities identified • What type of mistakes could occur • What is worst thing that could happen • What are defenses for worst case and typical mistakes • Have Lessons Learned been reviewed

  18. SWPF Safe Work Plan • SWP Acknowledgement • Each worker who will perform work on the assigned task must attend a briefing on the daily SWP and sign off below. Supervisors and workers sign the SWP to indicate their participation, understanding, and agreement to follow the requirements of the SWP. The signature on the SWP is a documented acknowledgement of work scope, hazard assessment, controls and safe plan. F. Lessons Learned Feedback This Shift/Activity G. Comments
