  3. Lets Get Started Off and Running !

  4. Advocate Case Loads • You should assign yourselves as advocates and start services on families as soon as the child is enrolled. • When a child leaves the program, you should stop services and remove yourselves as advocates. • Does everyone feel comfortable doing this? Lets look at a COPA task needing immediate attention!!

  5. Family Service Report • Due: By the 5th of each month. • We have a revised form • Lets look at the new form

  6. Parent Committee Meeting • Parent Meetings need to be fun and eventful !

  7. Parent Committee Meetings • Copies of parent meeting minutes should be submitted to my office by the 5th of the following month after the meeting occurs.

Parent Committee Meeting ( Cont.) • For the meeting documentation to be considered complete, you must send the following items: • Agenda (Completed) • Minutes ( That contain information) • Sign-in sheet All documentation should be completely filled in and if training was provided, the training and trainer should show up on all three pieces of documentation. • Let’s look at the forms

  9. Parent Committee Meeting (Cont.) Things to remember about parent meetings • You must have a parent committee meeting each month. • You must provide some type of training for your parents. • If you cancel a meeting, you must reschedule as soon as possible. • Each time a meeting is cancelled, you must submit a cancellation form. You will need to submit a form for each cancellation. • You must post copies of the meetings on your parent board. • If you have only one parent in attendance at a meeting, you still need to proceed with your meeting and possibly reschedule another meeting at a different time to reach other parents if time permits. • If your attendance is low at parent meetings or is nonexistent for one month, you should look at why. Brainstorm solutions with other staff to come up with ideas on what can be done to possibly correct the problem.

Parent Boards • Parent boards are the responsibility of all staff. • FSWs are responsible for monitoring the boards to ensure that boards are up to date and are a good source of information for parents. • Parent boards are a communication tool. They need to be designed with that in mind. • All parent boards will need to be monitored before the end of October. • The monitoring forms must be submitted to my office by November 5th with your monthly reports, if not before.

Contents of Family Service Worker Folders • Record of disclosure • Copy of child’s application • Classroom volunteer declaration of child abuse. • Family assessment • Family partnerships • Family survey • ALL family contacts • Each child’s records are to be kept individually. Please don’t keep all them in notebooks together. Each child MUST have a folder of his/her own.

  13. Health History • Health history must be completed before a child enters the classroom. • If a child has a chronic illness, then a special plan will need to be put in place. Let’s look at the health history on COPA

  14. Medical Record • There is information you need to include: • Medical Insurance • Up to date screenings • Medical home • Dental All areas of the medical record must be filled out. • NO BLANKS ! Lets look at the medical record

  15. Family Interest Survey • There are two purposes in this form. One is to assist with goal setting and referrals. The other is to provide feedback into the child’s nutrition needs. Things to remember: • Lori will need you to copy the back page and send it to her once it’s completed. • You will be responsible for keeping a copy of this document in your folder.

  16. Immunizations • Please remember to address the immunization status at the bottom of the immunization record on COPA. • As for our policy on immunizations, families need to be notified one month prior to expiration of immunization record. • The child can remain in the program two weeks after an expired immunization record. After that the child should be excluded from the program until an up to date record is brought in and loaded on COPA. This does not include Model City.

Family Assessments • Each family must have an assessment completed. • The first assessment should be completed within 60 calendar days after enrollment. • To the extent possible , the family assessment should be developed in a home setting. If this isn’t possible, a location should be agreed upon by you and the family. This does not relieve you from doing a home visit. A home visit should be scheduled with the parent as soon as possible. • If a parent refuses to let you do a home visit, the assessment would still need to be completed and it must be documented in case notes that the parent refused. There shouldn’t be a lot of these because you can remind parents that visits are part of our program polices

Family Assessment (Cont.) • The family assessment addresses eleven areas and each should be given equal time during the interview. • All areas should include good interview questions For example, if a family states that they have health insurance, the assessment should contain the name of the insurance. There should never be an area addressed with just “yes” and “no” answers. • When setting objective scores, you need to look at the chart provided in your packet. (pull from packet) • The second assessment should be done no earlier than 120 days after the first or no later than one month before school ends. • Once a family assessment has been loaded on COPA, a copy should be printed for the family folder to allow for easy follow up. • See Instruction Sheet in packets for more information

  19. Family Partnership Goals • For a goal to be considered a good goal, it must be: • S • M • A • R • T

  21. Family Partnership Goals (Cont.) • Family partnerships must be entered on COPA. • The yellow copy of the agreement should be kept in your folder and the white copy given to the parent.

  22. Family Partnership Goals (Cont) • Goals must be entered into with specific things in mind. They must be precise and detailed. • Goals must be measurable. That’s why your goals must be broken into steps. This will allow you and the parent to evaluate each step to determine if progress is being made and what support might be needed. • Goals must be agreed on. If you and your parent don’t agree on the goal, it becomes only your goal. • Goals must be reachable. There should be at least a 50/50 chance of reaching the goal. • Goals must be trackable. They must have a time frame in which to get things done. That’s why your goals must have timelines identified with the steps.

  23. Family Referral & Services • Referrals are a wonderful way to show your working with families. • They’re short and easy. You just load them on COPA. You don’t have to print anything. They will be monitored from there. • A family can have many referrals during the year depending on a family’s circumstances. • Keep in mind that if you refer a family out of the program to a health provider for services such as follow up on failed hearing, dental, vision, lead or TB skin test, these should be loaded into the referral section on COPA. • Lets look at referrals on COPA

  24. Staffing • The Staffing form is now located on COPA. You will find it under the “Checklist” tab on the child data sheet. • There will be no paper copies. • The staffing of children and families is the responsibility of teachers, family service workers and other staff that might need to provide information. • First staffing can be started during September but must be completed by the end of October. • Second staffing can be started during March and must be completed by the end of April. • There is a copy of the question/answer sheet in your folder. Keep this and refer to it during the staffing sessions if needed. • Lets look at the staffing form!

  25. Schedules • Let’s look at schedules. • Make sure I have your completed family service information sheet. • Make sure I have a copy of your parent books/ volunteer books . • Have a safe trip home !
