Scope of Problem • Estimates for 2012 • 1 in 50 Americans will get Melanoma • 76,250 invasive and 55,560 in situ cases • 9180 deaths (and rising) • 5th most common cancer of all • Greatest cancer killer among women age 25-35 • 10% of visits are for dermatologic issues • 70% of patients first see PCP for dermatology

The Way Things Were 2006 -Melanoma training at IUSOM • Two weeks of didactics: 2nd years • total < 10 hrs for 50% of students on Indy campus, test at end of course but no mandate to attend • One month Dermatology elective available for all 4th years • < 40% opt in • Informal and unreliable exposure on other clinical rotations

In order to rectify this shortfall, in 2007, with generous support from Outrun the Sun, Inc. and its donors to provide funding for improvement of Primary Care knowledge of Melanoma, IU Health Dermatology began developing a teaching program to make Melanoma a greater priority in the Medical Student curriculum

  6. Newly added 2009! • Objective Structured Clinical Exam: • 4th Year Students • Dermatology Elective is designated as a course which fulfills a core competency in “Lifelong Learning”

  7. Newly added 2011! • Web Based Medical Management Module: • 4th year Dermatology Elective • Required day in Melanoma Specialty Clinic: • 4th year Dermatology Elective

  8. A 35 y.o. man presents to your clinic on his wife’s request for evaluation of a mole on his back. The dark spot has been growing larger for many years. Over the past several months, it has had red bumps arising out of it.  It is otherwise asymptomatic. What would you recommend? • Wait and watch • Deep shave biopsy • Excisional biopsy • Fine needle aspiration • Black salve therapy

Immunotherapy • The encouraging responses to ipilimumab, IL-2 and interferon; known aggressiveness of melanoma in those with immunosuppression; and recent advances in knowledge of tumor biology have led to a strong investigational push into immunomodulatory therapy for metastatic melanoma • Unfortunately, none of the following have yet proven significantly successful or are only in early testing phases: • GM2 ganglioside vaccine • BCG vaccine • Melacine (melanoma antigen admixture) vaccine • GM-CSF infusions • CD 127 monoclonal antibody

  10. Program Goal Better educate medical students on dermatologic cancers with a particular emphasis on melanoma • Prevention • Detection • Management

Plan to Achieve This Goal Teaching modules for each year of training • PBL: 1st year • Traditional didactic: 2nd year • Surgical management of melanoma on-line (OnCourse): 3rd year • Medical management of melanoma on-line (OnCourse): 4th year • Objective Structured Clinical Exam (OSCE): 4th year • Package this for ALL US Medical Training Programs and convert to CME for already practicing MD’s
