Government legislation • The Government white paper “our health our care our say “ states that campus sites need to close by 2010 • 36 people will move in total, 7 people are still to move and 4 people will have a new provider. • This exciting new venture has been completed in partnership PCT’s, TEWV Trust, DBC Housing, Landlords and Support Providers and Advocacy networks. • A board of members have been instrumental in the delivery of this project with the implementation of a monthly project plan and report.

  3. What we identified • Campus settings limit choice and control for people with poor outcomes. • 3 Campus sites will close this year • 1 Supported living scheme will transfer their support from NHS staff to a Domiciliary Provider, for individual support. • Each person will have their own tenancy. • Each person has chosen who they want to live with. • Each person has had a full person centred review.

What we did • Created a board of multi agency members who meet monthly. • Developed the project in a project management style. • Understood the finance (Efficiencies), legalities, HR issues and the frameworks within. • 6 properties were purchased for people to choose where to live. • Each person made a choice of who they wanted to live with • 3 people have moved into their own tenancies with individual support

  6. What we have to do • 7 People are still to move from Tyne Tees and Wear. 3 people have chosen a new house. • Struan tenants will have their new provider in place by Nov 08. • 4 people are currently being supported to look for a new property on the open market. • Everyone will have moved by Dec 2008 • We are two years ahead of time. • An independent evaluation of this project with begin in Dec 2008, outcomes will be measured and the findings reported.
