Objectives • Overview of NHSN Patient Safety Component • Facility Administrator Designation • Enrollment • Five step process • Secure Data Network and digital certificates • Administrative set-up • Add users and assign rights to users • Add locations • Submit Monthly Reporting Plan • Join NYSDOH group and confer rights to NYSDOH to meet reporting obligations

Patient Safety Component • Device-associated Module • Central line-associated bloodstream infection • Catheter-associated urinary tract infection • Ventilator-associated pneumonia • Dialysis incident • Procedure-associated Module • Surgical site infection • Post-procedure pneumonia • Medication-associated Module • Antimicrobial use and resistance option

Facility Administrator Designation • One person who has primary functions for: • Enrolling facility • Registering/deleting users and determine user data access rights • Has ability to nominate and join Groups (data sharing arrangements such as NYSDOH group) • Other users can have administrative rights

  5. Facility Administrator Designation • Facility Administrator and all users must have: • Email address and Internet access on computers used for reporting • Must be able to download a digital certificate onto those computers

  6. Other Personnel Roles • NHSN Primary Contact Person • Person who interacts most closely with CDC regarding this component • An individual may have multiple roles • For example, the lead ICP may be the NHSN Facility Administrator and the NHSN Primary Contact Person

  7. Enrollment • Facility Administrator responsibilities • Review and accept Rules of Behavior • Register • Apply for and install an SDN digital certificate for NHSN Enrollment activity • Print, complete and submit online: • Facility Contact Information Form • Hospital Survey Form • Print, sign and return Consent Form to NHSN • Agree to follow protocols and use definitions exactly • Send acceptable PS Component data per NYS reporting requirements, including annual survey

Enrollment Process • Process • Upon receipt of the signed Consent Form, CDC will activate your hospital’s NHSN account and send an email to the Facility Administrator indicating that enrollment is complete • Facility Administrator can then access NHSN via the SDN by selecting NHSN Reporting activity

  12. Step 3: Go to SDN to apply for a digital certificate for NHSN Enrollment activity

What is the SDN? • SDN = Secure Data Network • Provides security control services to most CDC systems, including the NHSN • Physical and environmental controls – The computer room that houses NHSN is physically secure and environmental controls are used to protect NHSN computing resources from system damage or failure. • Network controls - The SDN is located behind a firewall and is protected by a centralized security gateway (proxy server). • User Authentication – All users must authenticate their identities with digital certificates

  14. What is a Digital Certificate? • A digital certificate provides an electronic means of proving your identity in order to securely conduct business with the NHSN. Digital certificates provide the following benefits: • Data is encrypted so only NHSN can read it • Provides assurance data is not changed in transit • Certifies who actually sent the transmission

  15. Digital Certificates • User specific • Don’t use anyone else’s and don’t let anyone else use yours! • Must be on each computer you use for NHSN access • Make a back-up copy Painful Problem Alert

  22. Digital Certificate • Once you receive your SDN email, you must have administrative rights to your computer to install your digital certificate. Get your IT staff involved. • When you are sure you have admin rights, from the computer you will use to access NHSN, click on the link in the email to install the digital certificate.

  23. Digital Certificate Once installed, you will get this screen: Make a back-up copy of your digital certificate

  24. Step 4: Enroll in NHSN • To log onto the NHSN via the SDN, go to: https://sdn.cdc.gov then enter your password **************

  29. After submission, await confirmatory email from NHSN Painful Problem Alert

  30. Step 5: Print, sign and return signed Consent Form to NHSN

  31. Primary contact: Don’t forget your signature! C-level signature required

  32. When NHSN receives your C-level signed consent form, they will activate your facility and send you an enrollment approval email

  33. To log onto the NHSN via the SDN, go to: https://sdn.cdc.gov Enter your SDN challenge phrase, click Submit **************

  35. Administrative Set-up:Add User and Assign Rights to Users

  36. Add User • NHSN Facility Administrator adds users • From Manage Users section of NHSN navigation bar, select Add • Enter data for user • Assign basic set of Patient Safety rights • Once you are more familiar with NHSN, if necessary, Customize Rights

Add User (cont.) • Once a user is added, NHSN will send an email with the following: • Agreement to follow the Rules of Behavior • Instructions on obtaining and downloading Digital Certificate • User can then access NHSN via the SDN and begin to enter data • See User Guide (tab 4) for details

  40. Add Locations • Decide which locations you wish to monitor. • At a minimum for 2007, add all critical care units (ICUs) and locations where colon and CABG (CBGB & CBGC) surgery patients may reside during inpatient stay • From the Facility section of NHSN Nav Bar, select Locations
