    9. The Metrics After reviewing history and assessing current health, each congregation is invited to set a goal for five measurable indicators of vitality. Goals should be prayer-centered, realistic, measurable and attainable goals based upon the assessment for each congregation.     11. The Strategic Plan HEALTH HISTORY – There are a multitude of health history assessments. Each conference or congregational development team may have an effective method for assessing a congregations health history. Percepts – GBGM Church Profile Assessment – Etc. (There will be an accessible health history report via the website.) Include information from the past 10 years of collected data. Current Health Assessment – Ministry Plan Tool available via the website. Again, conferences and congregational development directors/staff members have tools for helping a church assess their current situation. Please utilize these tools and encourage congregations to use these tools in order to discover their current health. The VCP Congregational Ministry Plan is available online as a multi-rater assessment. GOAL SETTING – Our Goal is to Make Disciples of Jesus Christ for the Transformation of the World. We measure our progress by looking at the baseline health vitality of each congregation. Based upon the history and the current health of a congregation, we invite each congregation to set a goal for their future health and vitality towards the mission of The United Methodist Church. Annual Conferences will set goals based upon the number of congregations setting goals. General Conference will set a goal based upon the number of Annual Conferences goals. Between 2012-2016 Agencies, Conference Staff, etc. will focus on redirecting attention, energy and resources on fostering, sustaining and increasing the number of vital congregations in our denomination.
