Highertheamountof alcoholconsumptionandlongeritis consumedbearsadirectrelationtothedevelopmentofcirrhosis.Menarelikelyto havecirrhosisthanwomenduetoahigheramountofalcoholconsumed.Women havetwicethehigherriskofdevelopingcirrhosisthanmen. • Stagesof AlcoholicLiverDisease • Thespectrumincludesasimplefattyliver, acutealcoholichepatitis,andalcoholic cirrhosis. Itisthe natural historyof thedisease but theoverlap can occur. • HepaticSteatosis orFattyLiver:Thereisan accumulation of fatin theliver. Thisstage is • reversible with alcoholabstinencei.e.quittingalcoholandmodifyingthe sedentarylifestyle toamoreactiveone.
AcuteAlcoholicHepatitis:Itisthestagein whichalcoholcausesinflammation oftheliver substance.Thereversibilityoftheprocessdependsupon thedamagealreadydone.Long termsurvivalin alcoholichepatitispatientswho stopconsumingalcoholissignificantly greaterthanthepatientswho continuetakingalcohol. • AlcoholicCirrhosis:Thisistheend-stage liver diseasewhichisconsideredtobe • irreversible.Itischaracterized bythescarring oftheliver.Itistheresult ofprolonged and severeliver damage. • Symptoms • The patientswithfattyliverare usuallyasymptomatic. • Alcoholichepatitisvariesin severityandsimilarlythesymptoms.Symptomscanbe • mildsuchasanorexia,weightloss,abdominaldistension,nausea,orvomiting.In severe cases,itcanpresentwithencephalopathy andliverfailure. • Alcoholiccirrhosispresentswithsignsandsymptomsofliverfailuresuchas jaundice,itching,lossofbodyhails,bleedingtendencies,hematemesis,ascites (collectionof fluid intheabdomen),and encephalopathy(sleeplessness,drowsiness, confusionandifsevereunconsciousness). • Diagnosis • Withthesuspicionofthediseasethetreatingdoctorwillbegetting– • Bloodtestssuch ascompletebloodcount,liverfunctiontest, coagulationprofile, andothers. • Ultrasound oftheabdomen or CTscan ofthe abdomenwillalsobedone asthe casemay be. • Aliver biopsymaysometimesbeneeded. • Fibroscanisalso helpfulin assessing theseverityandprogression ofthe disease. • Treatment • Themainstayofthetreatmentofalcoholicliverdiseaseisquittingalcohol.Quitting alcoholmaynotbe easyformany andthereforerequirespropercounselingand rehabilitationprogram.Patientswithseveresymptomsmayrequire hospitalization forworkup and management. Alcoholic hepatitispatientsusually have some nutritional deficiencies.Theyrequirevitaminsupplementationsandagoodprotein and caloricintake.
Livertransplantationis requiredforadvancedcirrhosiswithdecompensation (ascites,encephalopathy,jaundice,varicealbleed).Theearliertheoptionistaken the bettertheprognosis.
