  2. QurBook • QurBook is our one-stop shop mobile application for all patient healthcare needs. Here, patients can see their care regimen, adherence, vitals, appointments, health documents and so much more. • Patients can also talk to Sheela to get help or gain insight on their health journey. QurBook also enables access for caregivers in the app. This helps the whole family understand the steps to improving each other's health. Available on iOS and Android. 3

  3. How to Download the app 1. Open the play store or App Store and type “qurbook” app name with small letters without giving space. 2. Install the app as shown 4

  4. Signing Up with QurBook 1. Once the app is installed kindly Click on Signup option 5

  5. Sign up Screen Enter the following details: • First Name • Last Name • Phone Number • Email ID • Create New Password 6

  6. After sign up • After sign up you will receive an OTP to the registered mobile number • Enter the OTP and click on Verify option to enter the main screen Note:OTP should be entered in given time. 7

  7. Already sign up • If you have already sign up, you can enter the phone number and password • If you forgot the password, you could click on forgot password option to recreate the new password to log in. 8

New Password Creation Screen • Click on forgot password screen • You will receive an OTP to the registered mobile number • Enter the OTP and Create Your “NEW PASSWORD” • Click on “CHANGE PASSWORD” • If you are unable to receive the OTP Through SMS you can receive the OTP on call by clicking on “Confirm Via Call” 9

  9. Post Sign Up Main screen of Qurbook App has following options : • Care Plan • Regimen • Vitals • Symptoms • Family • Provider • Help desk • Chat with us 10

  10. 1. Care plan • Avail care plans to manage the health conditions effectively. Care plans are spread across 30 health categories to choose from 11

  11. 2. Dietplan • Choose among 450+ Diet plan that suits your need and compliments your health. Take dietitians consultation or customize your plan. Plans available as per region and preferences. 12

Regimen • Once the Plan is mapped, you can see that day scheduled and missed activities in the regimen option which you must perform for the day • You can Change the timings of the activities by clicking on the setting icon on right side of the screen • Once you do any activity for the day, you have to mention it in the regimen by clicking on check box. 13

  13. 4. Vitals and symptoms • You can enter their vitals sign like • Blood pressure • Sugar values • Weight • Pulse • Temperature • In Plan Symptom option, you can enter their symptoms from which they are suffering to maintain a record 14

  14. 5. Family Add their family member in the family option to extend the services to your loved ones. 15

Upload Prescriptions/ Reports & more in Qurbook app • You can upload the prescriptions/ Lab reports/ Medical records in the QurBook app by clicking on my records option which is in the bottom to the right side • You can also upload the copies and save. • You can also upload images from mobile gallery 16

  16. 7. Get to Know ‘Sheela’ • Sheela is the World’s first AI healthcare assistant. Sheela helps you and guides you at every single step with your activities, queries and tasks. • Activate Sheela • Step 1- Go to Home screen and click on the ‘Sheela’ icon at the bottom • Step 2- Sheela Screen will open in front of you. Click on ‘Start now’ 17

  17. 7. Get to Know ‘Sheela’ • Sheela will greet you in the next screen • Step 3-Click on the microphone icon “ ” on the bottom center and start conversing with Sheela • Ask your daily activity, and symptom or any question related to your health 18

  18. 8. Choose your preferred language for Sheela • You can also choose the preferred language to chat with Sheela by clicking on the Language icon “ ” on the top right corner 19

  19. 9. Contact Your Care Coordinator • Your can contact your personal care coordinator by sending a text message in “CHAT WITH US” option. 20
