    4. FSO Checklist FSO COVER SHEET FREQUENT ERROR AND MISSING INFORMATION CHECK LIST The government Security Program WILL NOT review your submission of the SF-85P. The investigating agency, the Office of Personnel Management, (OPM), will contact the company Facility Security Officer for corrections, or insufficient information. Errors and missing information delays the timely completion of the investigation. I have reviewed the 85P for_____________(name of employee) and it is complete.     5. FSO Checklist Ensure all blocks of information are complete. The most frequent error; missing information pertaining to city, state, and zip code. If this information is missing, or incomplete, your SF 85P will be returned for correction. __ Ensure the Social Security Number is on all pages of the SF 85P. __ Question #9; Where have your lived. Chronological record with NO gaps. __ Question #11; Employment dates from the past 7 years up to your current position. NO gaps. __ALL 3 signatures, dated, and full SSN. __ Question #13 CAN NOT BE FAMILY MEMBERS. __ Fingerprint Cards (SD 258) are completed and signed. This error will STOP the investigation, with no notice. __ FEDEX ENVELOPE WITH MAILING LABEL COMPLETED
