  1. Processes Did you ever feel like you would never get STISETS correct? Never really understand which processes to use and which forms are needed??

  2. Hopefully these PowerPoint Presentations will unlock the confusion and from this point on it will all be smooth sailing.

  3. To Access Processes you must: • Go to process manager under student desktop or • Double click on student name on your user desktop. • Either of these ways will take you to the process manager.

  4. Any forms used in sets must come through a process. • These are the possible processes available at this time.

  5. The following slides will cover the processes seldom used but available. • ECEC Process • Hearing Screening Process • Vision Screening Process • Record of Access Process • Observation Process 5 4 3 2 1

ECEC Process • This process contains only one form. • It is only used if a new ECEC checklist is needed for re-evaluation because there was not one done in the initial referral or if circumstances in a student’s life has drastically changed and new information is needed. • The first ECEC checklist is included in the initial referral.

  7. Hearing and Vision Process • Both of these processes are included in the drop down list but it is preferred that you do not use theseprocesses but rather the Shelby County Vision /Hearing form. • Results are reported on the REM form.

  8. This form is actually done automatically whenever anyone does anything in a student’s file. RECORD OF ACCESS PROCESS

  10. A paper copy of the observation is sufficient. Observation results must be entered on the REM form. OBSERVATION PROCESS

REMEMBER • ECEC Process • Hearing Screening Process • Vision Screening Process • Record of Access Process • Observation Process The above 5 processes are available but it is preferred that they would not be used regularly unless the paper forms are unavailable. It is not wrong to use them but tends to clutter the process manager and is unnecessary. The results of these processes are always included on the REM form.

  12. The Referral Process • This process is used for two different purposes. 1. It is always used for Initial Referrals. 2. It is used for out of state transfer students. • When writing the first IEP be sure and write it using the forms in this process. DO NOT ADD IEP PROCESS. • Be sure and complete all forms and process.

  13. The IEP Process • See IEP PowerPoint for in-depth details on how to write an IEP. • This process is used for the annual IEP. • It is NOT used for initial IEP. That IEP is done in the referral process. • A new IEP form will not be allowed until previous IEP Process is completed. Always complete all forms and processes.

  14. The Reevaluation for Eligibility Process • This process is used for two different purposes. • 1.This process is used for the 3 year reevaluation • 2. Used for any reevaluation conducted for the purpose of determining eligibility. • If new IEP is written as a result f this reevaluation, the IEP forms in this process are used. DO NOT ADD IEP PROCESS. • Complete all forms used and the process.

  15. The Reevaluation for IEP Changes Process • Used to find additional data to aid in the writing of an appropriate IEP. • It does not involve any eligibility decision. • Is used for Adaptive PE, OT, PT evaluation. • An IEP must be conducted when testing is completed. • NO ELIGIBILITY MEETING is held.

  16. Post-Transition Survey • Used for students who have graduated

  17. Revocation of Services • This is a new process to be used when parent’s of active students who are still eligible are removed from Special Education by parents. • Teachers must open this process and fill out the form for parents to sign. • Complete the process after it is signed.

  18. Summary of Academic Achievement and Functional Performance The purpose of this process is to: 1. document academic achievement and functional performance progress. 2. document recommendations for meeting the goal(s) for postsecondary education/employment or community /independent living goals not met. 3. document successful accommodations in high school. Must be done for every exiting senior or for those who will age out of school.

  19. Summary of Academic Achievement and Functional Performance • The case manager is responsible for providing the complete SOP, including the cover page, the transition goal progress report, student input page, summary of progress and accommodations and copy of high school grades/credits upon exit from high school. • Place a copy of all the above completed documentation in each exiting student file.
