Theyareinterestedinthevarietyofdiagnosesthatweseeincludingthecomplexsurgicalandnon surgicalcases.Theyarealwayssurprised,though,whenItellthemtheagesofthepatientsthat come.Theyseemshockedthatsomeofmypatientscanbeasyoungas9to11yearsold. Thesepatientsdopresentwiththecommonfracturesandsprainsfromvarioussportsandjustbeing kids.However,wealsoseeafairamountofpatientsthisagerangethatarehavingproblemsdueto overtrainingandoveruse.Onesportthatwecanseethisinisbaseball. Thereareavarietyofoveruseinjuriesthatoccurinbaseballinvolvingmultiplejoints.TodayIwould liketotalkaboutLittleLeagueElbow. Thereisageneralconsensusthattheinjuryrateinchildrenisincreasing.Ihaveseenthisinmytime asaphysical therapistoverthepast8years.Onereasonthatthishasoccurredismoresport specializationearlieroninachild’sathleticcareer.Thismeansthatinsteadofplayinganywherefrom 2-4differentsportsayear,theyareplayingthesamesport,onmultipleteams,fortheentireyear.This canresultinoveruse,andnotenoughvariabilityinhowachildmovesorstresseshis/herbody.This thencanleadtostressrelatedoveruseinjuries. LittleLeagueelbowdescribesa “lesiontothemedialaspectoftheelbow” (1) andisusuallycaused secondarytoexcessstressplacedacrosstheelbowduringtheearlyphasesofthrowingincluding thecockingphase.Theelbowcanbebothimpactedontheinsideaspectduetoadistractingforce ortheoutsideoftheelbowduetoacompressionforce.Therefore,symptomscanpreventitselfina varietyofways. Anefficientevaluationandexamisrequiredtoensureappropriatediagnosesandimagingisoften performed.Dependingontheathlete’sagetheymaypresentwithapophysitis (inflammationofthe growthplates) oranepicondylaravulsionfracturewheretheligamentpullspartoftheboneaway fromthemainstructure (2)

Wheneverayoungthrowingathletecomplainsofmedialelbowpain,LittleLeagueElbowwillbeWheneverayoungthrowingathletecomplainsofmedialelbowpain,LittleLeagueElbowwillbe consideredmorelikely.Theearlierthisisspottedandaddressed,thebetterthethrowersprognosisis withareducedriskoffunctionallimitationsordamage (1) PhysicaltherapyisaneffectivetoolinrecoveringfromLittleLeagueElbow.Itisimportant,asitisan overuseinjury,toremovetheaggravatingfactor,whichistypicallythrowingforatleast4-6weeks (2). Thegoalofphysicaltherapyistorestorenormalmotion,improvetheathletescoreandshoulder strength,improvethethrowersunderstandingofhowtousemorethanjusttheirarmtothrow,and educatetheparentsandcoachesonhowtopreventthesetypesofinjuries. Howdoesonepreventthesetypesofinjuries?Encouraginggoodthrowingmechanicsatayoung ageisakeypartofreducingthelikelihoodofthesetypesofinjuries (1).Followingaspecificpitch countandnotthrowingfatiguedisalsoanimportantaspectofminimizingtheeffectsofoveruse.The physiciansandphysicaltherapistshouldnotbetheonlyoneresponsibleforthehealthandwell beingoftheseyoungathletes.Coachesandparentsneedtotakeonthisresponsibilityaswelland recognizewhentheirathletesandkidsareshowingsignsofdiscomfort,fatigue,andpoor mechanics.
