Capacity & Coordination (based upon Scope of Service of funding) Membership Responsibilities of PHAC: • Each municipal & county government • Each community hospital • Each SAU • Each community and mental health center • Each CDC community sector • Other agencies designated by the Public Health Advisory Committee • The contractor • Develop mission statement • Develop annual priorities, goals & objectives • Develop an organizational chart • Develop geographic-based region name • Develop committee policies & procedures • Develop committees & workgroups • Provide oversight for contracted public health services

Performance Measures (based upon Scope of Services) • Representation by 70% of CDC 11 community sectors • Representation by 65% of Governor’s Commission 6 community sectors • Representation by 70% of DPHS 13 healthcare sector partners • Documented organizational structure (e.g. vision or mission statements, organizational charts, MOUs, minutes, etc.) • Increased stakeholder connectivity and improved trust among partners (as demonstrated by annual PARTNER Survey)

Public Health Advisory Committee Purpose & Goal ( according to Scope of Service in grant) Develop a Regional Public Health Advisory Committee, comprised of leaders from key community sectors, to advise the contractor on Public Health Network services and build additional capacity among regional partners to protect and promote the health of their communities.

Regional Meeting Schedule • PHAC: Quarterly (Oct/Jan/Apr/July) 4th Thurs @ 9-10:30 am • Preparedness Network: Bi-Monthly (Sept.) 2nd Wed @ 12-2 pm • Healthcare Network w/Greater Derry: Bi-monthly (Oct) 3rd Thurs @ 1-3 pm • Prevention Round-Table: Bi-monthly (Sept.) 4th Tues @ 3:00-4:30 pm • ASAP Task Force: Quarterly (Oct/Jan/Apr/July) 3rd Tues @ 1-2:30 pm • Statewide Substance Misuse Prevention Coordinators Mtg: Bi-monthly (Sept) 3rd Wed 9-4 pm and Public Health Coordinators: Bi-monthly 1st Tuesday, October 1
