Use visual / audible cues HAPU Driver Diagram Standardized pressure ulcer risk assessment for all patients within 24 hours of admission Initial Skin Assessment Upon Admission Reassess daily or more frequently for high-risk patietns Daily Skin Reassessment Adopt a head-to-toe daily skin and risk assessment tool Secondary Driver Manage Moisture Zero Stage III or IV HAPU Keep skin dry and hydrated. Sequence implementation by drug class Optimize Hydration and Nutrition Assess weight, nutrition and hydration status Develop and institute early mobility/ambulation protocols Minimize Pressure Turn and reposition patients every two hours. Ensure pressure-reducing equipment is available at all times
