Responsibilities Include • Emergency Management and Continuity Planning for the UCLA Health System, Faculty Practice Group, and School of Medicine • Exercises, Education, and Trainings • Disaster Plans, Manuals, and Kits • Evacuation Procedures and Devices • Disaster Assistance Response Team (DART) Program • Decontamination Team • Community Emergency Response Team (CERT) • Personal Preparedness

Disaster Activation Notification and Responsibilities On-Campus • Code Triage Internal or External Level I • Overhead Announcement • Department Head Page • Responsibilities • Provide for safety of patients, staff, and visitors; • Maintain situational awareness by listening to overhead announcements and directions including safety messages • Staff are not permitted to go home unless otherwise directed by the Incident Commander or Supervisor. • Inventory staff, supplies, and resources; • Input your unit information into the Unit Disaster Report Form (UDAR) or online via command aware at www.commandaware.com.

Disaster Activation Notification and Responsibilities Off-Campus • Call disaster hotline: • SMUCLA: 424-259-9999 • RRUCLA: 310-206-3232 • If notified and available, indicate estimated time of arrival and report for duty; • Wear staff identification badge at all times; • Report to the Labor Pool for assignment; • Be prepared to stay for an extended amount of time (bring an extra change of clothes, toothbrush ect.)

Hospital Incident Command System • Utilization of Hospital Incident Command System (HICS) • Managed from the Hospital Command Center • Centralized decision-making and coordination of resources • The Incident Commander assumes authority and responsibility for the management of the entire incident.
