Netwerkbijeenkomst • Goede onderlinge samenwerking ketenzorg dementie Waardenland • Rivas initiatiefnemer van belangrijk thema binnen de ketenzorg dementie: domotica! • Sluit aan bij jaarthema Drechtzorg: zelfmanagement en zelfregie. • Belang om deelnemers aan de netwerkbijeenkomst onderling informatie uit te laten wisselen en handvatten te bieden bij interne en gezamenlijke vernieuwingen.

Drechtzorg in het kort • Stichting is in 2000 opgericht door zorgaanbieders in regio Dordrecht. • Transmuraal samenwerkingsverband en netwerkorganisatie. • Doelstelling: stimuleert en ondersteunt initiatieven, activiteiten en projecten gericht op samenhang, afstemming en continuïteit van de zorg. Oftewel: • De juiste zorg voor de juiste client op de juiste plek met de juiste hulpverlener.
