Submission Requirements for School-Based Management General Assembly in Bacolod City

In preparation for the upcoming School-Based Management General Assembly in Bacolod City, each division is required to submit specific reports by January 6, 2012, addressed to FED. The reports include FYs 2009 and 2010 SBM Grants Utilization Reports, FYs 2009 and 2010 Accomplishment Reports, a consolidated SBM Level of Practice report for SYs 2009-2012 with a list of schools and IDs, and the Accomplishment of the Scoring Template. Timely submission is essential to ensure a successful assembly and reflect the performance of the divisions.

School Based • In preparation for the forthcoming School - Based Management General Assembly in Bacolod City, each Division is required to submit the following reports on or before January 6, 2012 c/o FED: • a. FYs 2009 & 2010 SBM Grants Utilization Reports; • b. FYs 2009& 2010 Accomplishment Reports • c. SYs 2009- 2010, 2010-2011 & 2011-2012 SBM Level of Practice (consolidated report by division with corresponding list of schools and school IDs. • d. Accomplishment of the Scoring Template
