  3. 2010-2011 Process and Procedures • Non-Participation Waivers: • SA did not compete at ALL but was enrolled. • No waiver needed if SA was NOT enrolled • Submit the following: • Ivy League Record of Injury (if applicable) • Ivy League Waiver Form – Check appropriate box (new) • Medical Documentation

  4. 2010-2011 Process and Procedures • NCAA Medical Hardship Waiver • SA competed in competition, scrimmage, and/or non-traditional-season contest • Submit the following: • Ivy League Record of Injury • Ivy League Waiver Form (check appropriate box) • For individual sports attach calculation of first half of season calendar (provided calendars) • Participation Records • Schedule • Medical Documentation

  5. 2010-2011 Process and Procedures • Ivy League 5th Year Waiver • SA will be enrolled during a 9th semester (5th year) or beyond and would like to use remaining athletic eligibility. • Transfer SAs do not need a 5th year waiver unless their enrollment is extended after entry at an Ivy institution. • Waivers DUE June 30 prior to 5th year • Updated Waiver Form • Institutional list of waivers approved for S-As in class of 2011

  6. Delays in Waiver Approval • Record of Injury not signed by institutional physician • No schedule or participation record included • Calculation of mid-point

  7. Waivers Involving Mental Illness • Hardship or non-participation waivers involving mental illness. • Must be documented just like an injury – SA must see a psychologist or psychiatrist and must submit contemporaneous medical documentation. • Office visit, office notes, • Diagnosis • Time of illness or injury or missed season

  8. Outside Competition • Tracking on outside competition – including competing unattached. • Why? • Interpretation – NCAA Staff April 13, 1994 • SA does not qualify for a hardship waiver if they compete in outside competition or compete unattached in the second half of the season. • Outside competition does not count in calculating the minimum participation (30%).

  9. Ivy Office Procedures • Please fill out form for our records regarding letters. • Future upgrades • Ability to enter SA data online • Send medical documentation by email or paper • Reports of hardship/non-participation waivers • Reports of 5th year waivers
