  3. Accommodations In compliance with the Americans with Disabilities Act (ADA), students must apply for and be granted specific accommodations. The nature of a student’s disability is confidential.

  4. Accommodations Note-taking assistance Alternative format texts Use of laptop in class Permission to record lectures Distraction-reduced environment for test-taking Extended test-taking time Letters of Consideration

  5. Goldman Office of Disability ServicesTesting Center • Test proctoring for students with appropriate registered accommodations. • 1500-2000 exams proctored per year. • Hours are adjusted on an as-needed basis to accommodate testing.

  6. How do students arrange for taking tests at the ODS Testing Center? Apply & activate accommodations. Present Course Accommodation Form (CAF) to professors. Complete Blue Sheet with professor. Professor arranges for test to be delivered to ODS on or before the day of the scheduled exam.

  7. Course Accommodation Form Student’s Name Splash ID Number Classroom Accommodations Testing Accommodations Signed by Director or Asst. Director of ODS • Student’s should request a private meeting to discuss this form. • Students will give you a copy of the form as a receipt. Your Signature

  8. Exam Request Form(a.k.a. “The Blue Sheet”) • Instructor to complete with student. • Start time may need to be adjusted to work with student’s other scheduled courses Student to Complete Please notify us if any materials are permitted. Tell us how you would like the test returned. Don’t forget to sign & date!

  9. Short-Term Services • Now offering support for students experiencing temporary conditions or injuries including: • On-campus transportation to classes • Other accommodations appropriate to condition
