  1. What is Case Reflection ? • Case reflection is one of the evaluation tools in this course. • Case reflection is a learning tool which helps to reflect on your own practices and • Identify gaps in the knowledge and attitude in your patient care, approach and planning • Thus enhance the insight and capacity to care which in turn benefit both you and the patient.

  2. Steps … • Identify your patient.  • Select a patient whom you have cared for in the past.  • The ideal patient to select is one for whome the management was not straight forward. • Do this as early as you can you have enough time to think about and write up your case reflection.

  3. Rules • Reflection of the total care of a patient and family. • identify situations that challenged either your knowledge or you personally. • At most importance to be given to confidentiality • The case reflection must be presented clearly in an unambiguous manner.

  4. Rules again • Identify yourself ONLYon the title page . • Avoid sentences expressing your gratitude to people who have helped you as these may revela your identity. • Do support your reflections with accurate literature references wherever possible. • While concluding, you should provide evidence of change in practice as a result of new learning.  • The word limit is 2000 words

  5. Format • The case reflection must be presented as soft copy in MSWord or in a compatible format only. If not it will be returned to you. • Preferred for is Arial and size 12. • Please do not send in other formats including odt, pdf or image files.

Role of a Mentor • Seek help from a mentor • Mentor can help you in the following: • Identifying the right patient • Helping you to prioritise issues and act as a ‘sounding board’ to guide your reflections. • Guiding you to organise and write down the reflections about the patient management both at the personal level and at the professional level

  7. Steps of writing a Case Reflection

  8. Step I : Introduction What is the nature of your work? Please do not specify your work place or centre This section is to help the faculty to assess your reflection in the proper perspective - whether you are a doctor/nurse and your role in the team. Maximum Marks: 5

  9. Step 2: Case Summary • What was the diagnosis? • Enumerate important clinical features, including physical findings. •  What investigations did you do and why? Complete the table . No details needed.

  10. Step 2 Continued… • What treatment and other management plans did you consider and why? •  What was the outcome? Was the outcome expected / anticipated? •  Who else did you involve or consult in the care of this patient? •  Were their contributions helpful? If so how?

Step 3: Candidate’s learning of Palliative Care Principles and Practice • Now that you have completed the training in Palliative Care. • Summarize briefly how you would have managed this patient with your new understanding of palliative care • What changes this new learning has brought to you at a personal level? • What changes this new learning has brought to you at a professional level? • What improvement this new learning has brought to you to your existing knowledge and skills in this area?

Candidate’s Learning • Explain briefly the improvement that has occurred in your knowledge and skills as a result of this new learning The responses can be grouped under: • Physical care • Psychosocial care • Spiritual care • Issues of communication with patient and family • Ethical issues if present

  14. Step 3Learning outcome – Word count • This section 3 should be of a minimum of 500 words • The total word count of the entire Case Reflection should not exceed 2000 words.

Case reflection continued • Case reflection to be submitted only as soft copy to the Centre Coordinator • Reflections forwarded by the candidates directly to the Chairperson or IAPC will not be accepted. • The soft copies submitted by the candidate should be forwarded to the Chairperson, Academics on or before 30th Dec 2014 • This will then be send to National Faculty for evaluation

  22. Please note the date • The evaluation of Case Reflections reaching the Chairperson, Academics IAPC after 30th Dec will be deferred and done along with the next batch. • The final results will be published on the website of IAPC in Feb 2015
