  3. Community Outreach to Reduce health Disparities-”Tell A Friend "About Mammogram Screenings Project Overview and Outcomes REACH US Regional Conference August 22nd-24 Presented By: Darryl Cannady Executive Director South Central Educational Development, Inc.

Community Outreach to Reduce health Disparities-”Tell A Friend "About Mammogram Screenings Project-CORD, a group of African American churches working to reduce health disparities in southern West Virginia. Curriculum Adapted from Seattle Department of Health. Target Population: African American women age 18-65 living in rural and underserved communities of Southern West Virginia. • McDowell and Mercer Counties.

  5. “Tell A Friend” Intervention Goals To Promote access to the high quality, evidence-based Cancer trainings Targeting underserved African Americans women cancer patients, and cancer survivors. 2. ToImprove access to and utilization of evidence-based cancer training for 14 to 20 rural underserved African American women Leaders living in southern West Virginia, including those who are under- or un-insured to increase access to cancer screenings and care.

  6. “Tell A Friend” Intervention Goals 3. To Educate health care professionals, and students about cultural sensitivity and cancer survivorship through providing workshops at the Minority Health Disparities in Rural Appalachia Conference on “Tell A Friend” About Mammogram Screenings.

  7. Objectives 1 • Recruit10-20 community leaders and stakeholders, church members, health care professionals, and survivors to participate in a community advisory board in McDowell and Mercer counties. • Measured By: • The number of participants attending meetings • Participants sign in sheets • Status: Complete recruitment of participants from McDowell and Mercer Counties.

Objective 2 • Recruit, educate/train 12 to 24 female cancer survivors as Role Models utilizing the “Tell A Friend” Adaptation of a model research tested intervention program, targeting local churches and inviting survivors to participate in the program. • Measured by: number of registration forms, sign in sheets, Training evaluation forms, Pre and Post Knowledge Retention Test. • Status: Complete • 60 Letters of invitation to churches and community groups • 4 Church Responses and 1 social group • 12 African American women participated in the leaders training.

Objective 2, Leaders Training • Leaders Training of “Tell A Friend” About Mammograms Screenings. • Pre Test (base line Knowledge). • Local Cancer Statistic. • Facilitation Skills utilizing Visual Tools (DVD). • Community Education Program Tools (CD). • Breast Health Education Resources. • Presentation and overview of the West Virginia Breast and Cervical Cancer Screening Program Free Services. • Referral to Care Systems • Post Test

Workshop Outcomes. • 110 Community Residents Participated in the Workshops • 84 African American Women • 17 Caucasian Women • 6 Hispanic Women • 3 Men • Referrals to the West Virginia Breast and Cervical Cancer Program Free Screening. • 22 Women from McDowell County • 18 Women from Mercer County

Evaluations • 90 % of the participants stated “I feel good about my ability to apply the new skills and information in teaching others”. • 96.7% of the participants stated “The workshop activities were helpful in applying the information”. • 97.8 % of the participants stated “I learned of resources for information, screening, and treatment”.

Evaluation Question • What did you like most about “Tell A Friend”? • The information • I could tell many friends about the program • Learning more about health care and taking care of yourself • Knowing what to say and how to say it • Highly informative and well organized. The food was fantastic and served before the workshop which was a great idea • It was very informative and I get to understand the topic better and very interactive • The environment of the workshop was very comfortable, encouraging • The information and handouts will be very helpful - knowing how to take care of your body and verbalize it to the doctor
