Shifting the Focus • Shifting the Focus Strategy, launched October 2007 • Aims • Ensure future outputs from NHS QIS are useable in community and primary healthcare • Focus on implementation and improvement – making sure that NHS QIS work programmes add value in community and primary healthcare • Build on the learning from the Scottish Patient Safety Programme

Shifting the Focus • Delphi Consultation Pilot (May to December 2008) • Key opportunities to support quality Improvement in community & primary healthcare Outputs from pilot: • The Patient Journey and Referral Management and Processes • Quality Assuring good practice and facilitating spread • Quality Assuring community pharmacy practice • Standards/Guidelines for tackling health inequalities • Supporting CHPs to deliver outcomes rather than setting standards

Shifting the Focus • CHPs/Shifting the Balance of Care Workstream (January 2009) • Shifting the Focus (stage 2) • Two year launch-pad project • build on the outputs from the Delphi Consultation Pilot – initial phase of the project • scope potential areas and opportunities where NHS QIS support might add best value • begin the process of putting in place quality improvement support mechanisms to assist CHPs in delivering agreed outcomes and improving outcomes

Shifting the Focus CHPs/Shifting the Balance of Care Workstream • Key Aims (Inward and Outward facing) • Make sure NHS QIS work programmes are meaningful and add value to CHP (and community and primary healthcare) priorities • Work with the Association of CHPs to build Network support • Shifting the Balance of Care Improvement and Resource Framework – aligning NHS QIS strategic objectives and work programmes • Partnership working with NES and NHS Health Scotland • Strategic links with Scottish Government Improvement and Support Programmes (Long Term Conditions, Mental Health and 18 Weeks) • Develop and share a Shifting the Focus Stage 2 work programme by December 2009

Shifting the Focus Shifting the Focus Stage 2 work programme • Key activity in 2009 • Outputs from Delphi Consultation Pilot represents initial phase - report published March 2009 • Further consultation/engagement with CHP General Managers and Clinical Leads – directly and through the Association of CHPs • Consult with NHS Board SBC Leads and Liaison Co-ordinator Networks • Establish strategic links with Scottish Government – including SBC Delivery Group, Primary Care Policy Leads and IST Programme Leads • Alignment of NHS QIS strategic objectives and work programmes with the 8 SBC Improvement Areas – this will also involve joint working with NES and NHS Health Scotland
