What is a demonstrator? • We had an idea of how to share information better and how to do this between RMBC Adult Care, Pennine Care NHS Foundation Trust, voluntary sector (e.g. Age Concern) and supporting people providers • We have been given an opportunity to try this out • The aim is to share what works well and for this to be replicable nationally

Rochdale’s Bid • Primary objective to improve service users’ experience and outcomes by: • Developing streamlined assessments and service delivery processes; • Building on existing integrated working practices; • Improving service efficiency; • Improving information sharing supported by technology.

  5. Core Components of Project • CAF messaging between Adult Care and Mental Health (RMBC – IAS, Pennine Care - Lorenzo); • Scoping out the streamlining of processes with Supporting People; • Third Sector Assessment Tool – Electronic on line referral to Adult Care; And an extra : • PROTOCOL 360 – Proactive alerting when combinations of risk factors are recorded on disparate IT systems.

Benefits locally • Reduced paperwork – more client contact • Up to date assessment detail so all involved are aware of the history • ONE integrated care and support plan • Reduce crisis and emergency care – through better Care and Support planning Increase appropriate client contact • Improved quality of service • Service user empowerment (though this more person-centre approach and being given access to copies of assessment and care plans)

  12. Summary • Streamline referral, assessment and service delivery process by sharing information more effectively; • Will be able to feed into and possibly influence national policy (Supporting People, Carers etc).
