CHCW 5 Years Ago • No dental services, lack of corporate efforts • One-on-one efforts, 1 RN/team, open access scheduling • QA: Introductions to PDSA cycle • Patients: Open access scheduling, Standard clinic hours, limited same day availability • Staff: Lots of longevity, growth, attract people who want to care for the underserved • Typical patient visit

Journey to PCMH • CHCW PCMH practice transformation journey • CHCW goal: submit corporate application by March 31, 2013 • Weekly Meetings: QA/Risk Director, COO, Care Manager and Clinic Managers • Worked on bringing awareness to staff • Learned how to use the points, chapter summary spreadsheet to breakdown tasks, Meaningful Use

  5. CHCW Now… • Bigger, better care teams! • Using staff to their license. More tasks, more delegation, EMR Implementation • QA Projects: Meaningful Use, Cervical Cancer Screening, CINA Report, EMR documentation, • Patience: Better Access, Improved forms, Practice Teams • Staffing: Longevity, involving staff in pilot projects • Patient Visit: Patient goal setting, group classes/ visits, improved access
