Job Description Review, Recommendation & Approval: Job Title: UPN: SOC/DOTCode: Division: Section: Location: Shift: Reports To: Grade/Step: SalaryLevel: Diabetes Prevention Data Entry/Recruiter SDP-05 Received byHR:7/19/2007 Recommendedby PersonnelCommittee:7/20/2007 7/30/2007 7/31/2007 Recommendedby FinanceCommittee: Approved by Board of Directors: Review&RecommendbyGDC: CommunityServices DiabetesProgram Potawot HealthVillage, Arcata Monday – Friday8a.m.-5p.m. DiabetesProgramManager Grade05-01to08-10 Level I – Grade 5 ($9.39/hr to $11.80/hror$19,531/yrto$24,544/yr) LevelII–Grade6-7($10.51/hrto$14.51/hror$21,861/yrto$30,181/yr) LevelIII–Grade8($12.55/hrto$15.93/hror$26,104/yrto$33,134/yr) Non-Exempt-Hourly July 31, 2007 July 19, 2007 FLSA Status: ApprovedOn: RevisedOn: • SUMMARY • The Diabetes Prevention Recruiter will be responsibleforoverseeingthe collectionandtransferof all projectand evaluationdataatallprojectsitesparticipating in theUIHSSDPICompetitiveGrants ProgramDiabetesPreventionDemonstrationProject.Thesedutieswill becarriedoutinaccordance withgrantguidelines, projectprotocols,andpoliciesandproceduresof UIHS.Thispersonwill workwith membersoftheinterdisciplinarydiabetesteamandparticipateindiabetesprogramteambuilding activities;andwillalsoassistwith programclericalduties:typing,wordprocessing, filing,copying.This isagrantfundedposition. • ESSENTIALDUTIESANDRESPONSIBILITIESincludethefollowing.Otherdutiesmaybeassigned. • Working underthedirectionoftheDiabetesProgramManagerandwithguidancefromtheDataCoreof theCGPCoordinatingCenter,theSiteDataEntry/Recruiterwill: • Collect datafromvarioussources,suchasinterviews,in-personcontact withparticipants, diabetesregistry,andpatientmedicalrecords. • Develop full understanding and skill in use of all protocols,forallaspectsofthedemonstration project;ensurethatdataiscollected,managed,andtransferredaccurately,ontime. • With the DiabetesProgramManagercreatetimelinesto completeproject objectives. • Assist in preparingprogressanddatareports;assistinensuringtheiraccuracyandtimelinessin submissiontotribalreview,IHS,andtheCoordinatingCenter. • Work with theDiabetesProgramManagertoensurethatalldatacollectionandmanagement activitiesareperformedwiththeutmostattentiontopatientconfidentialityandHIPAAandIRB requirements. • Participate activelyinall national,regional,andlocalgrantee projectdevelopmentandtraining meetings. Maintain collaborative relationships with grantee teammembers,CoordinatingCenterData Corestaff,technicalassistanceexpertconsultants,tribalandcommunity partners,andother projectstakeholderstofostersuccessfulproject developmentandimplementation. • Records minutes of meetings as assigned. • Participate in recruitment activities includinggroupandindividualpresentationsinthe clinicand communitysetting. • Maintains interpersonal communicationwith,participants.Coordinatesfollow-upofparticipants whodonotkeepscheduledappointments. • Maintains filing system of all project healthrecords.Interactswithprojectparticipantstomake datacollectionnon-threatening.Keepsschedulingflexibletomeetdatacollectiontimeframefor individualprojectparticipants. • If necessary accompaniesPhysical ActivityLifestyleSpecialistsinthefield. • Performs other duties asassigned. • SUPERVISORYRESPONSIBILITIES: • This positionhasnosupervisoryresponsibilities. • QUALIFICATIONS: • To perform this job successfully, an individual must be able toperformeachessentialduty satisfactorily.Therequirementslistedbelowarerepresentativeoftheknowledge,skill,and/orability required.Reasonableaccommodationsmaybemadetoenableindividualswithdisabilitiestoperform theessentialfunctions. • EDUCATION and/or EXPERIENCE: • Level I - High School diplomaorequivalentandone(1)yearrelatedclericalandadministrative assistantexperienceand/ortraining. • Level II - High School diploma or equivalent and four (4) years related experience. • Level III - Associatedegree(AA)andtwo(2)yearsdirectexperience;orHSdiplomaor equivalentandsix(6)yearsdirectexperience. • LANGUAGESKILLS: • Ability to read, analyze, and interpretgeneralbusinessperiodicals,professional journals, technicalprocedures,or governmentalregulations. • Ability to write reports, business correspondence,andproceduremanuals. • Ability to effectively present information and respondtoquestionsfromgroupsofmanagers, clients, customers,andthegeneralpublic. • MATHEMATICAL SKILLS: • Ability to calculatefiguresandamountsandtoworkwithspreadsheetformulas. • REASONINGABILITY: • Ability to define problems, collectdata,establish facts,anddrawvalidconclusions. • Ability to interpretanextensivevarietyoftechnicalinstructionsinmathematicalordiagramform anddealwithseveralabstractandconcretevariables. • CERTIFICATES, LICENSES, REGISTRATIONS: • Shall possessavaliddriver’slicense. Must be eligiblefortheagency’svehicleinsuranceandprovideproofofprivatevehicle coverage. • OTHERSKILLSANDABILITIES: • Ability to operate standardofficeequipment(typewrite,copier,fax,etc.). • Ability to operate computer and standardofficesoftware. Abilitytotype40wpm. • Ability to work with little supervision. • Ability to communicateeffectivelybothorallyandinwriting. • Must be able to work with American Indian People and be sensitivetotheirculture. • DemonstratedproficiencyinMicrosoftOfficeSuiteincluding:Word,Access,Excel, andPower Point.Experiencewithelectronicmedicalrecordssystemsdesirable.In additionademonstrated proficiencyinmeetingtranscriptionornotetakingandminute’spreparationisrequired.
