  3. DOH-Escambia Mission To protect, promote, and improve the health of all people in Florida through integrated state, county, and community efforts.

  4. Homeless Outreach Initiative: Overview A three phase approach to increase delivery of health information to people experiencing homelessness.

  5. Outreach Activities • Complete a 15-question agency survey • Allow us to administer a 13-question survey to your clients • Provide space and time for the homeless community survey at your agency

  6. Benefits • Designation as a DOH-Escambia partner for the Homeless Outreach Initiative • Press coverage for participating in the initiative • Final report that will include findings from the agency and homeless surveys • Clients will receive incentives for completing the survey

  7. How to be Involved • Schedule a meeting with the Homeless Outreach Coordinator
