Personal Health Assessment • You and your spouse complete questionnaire • Online beginning January 1, 2009 • Must be completed by December 15, 2009 • Receive results in less than 1 minute • Work with a a health coach to manage your health risks (voluntary) • Confidential – only the H&PM vendor will have access to responses • $200 premium contribution will be WAIVED if you and your spouse take the assessment

Healthy Heart Program Healthy Aging Program Diabetes-Fighting Program Cancer-Fighting Program Easy Start Program Smoke Free Program Healthier Diet Program Get in Shape Program Weight Loss Program Stress Relief Program H&PM Wellness Activity:Healthy Living Programs • Eligible employees volunteer • Complete 6 week program online • Must complete 3 programs – total of 18 weeks • Cannot do programs concurrently

Weight Management Stress Management Cholesterol Management Blood Pressure Management Smoking Cessation Nutrition Exercise Back Care H&PM Wellness Activity:Mail Based Lifestyle Management • Eligible employees invited based on results of personal health assessment and readiness to change • Voluntary participation • Receive 6 mailings and complete survey; 6 month program • Must combine with 2 Healthy Living programs • May do a Healthy Living program concurrent with mail-based program

Weight Management Stress Management Cholesterol Management Blood Pressure Management H&PM Lifestyle Management • Eligible employees are identified as high risk - from personal health assessment • Phone coaching by Personal Health Coach • 3 phone calls and reach short-term goal • 8-12 month program depending on risk level and readiness to change • Smoking Cessation • Nutrition • Exercise • Back Care

H&PM Wellness Activity:Care Management Program • Eligible employees are invited to enroll based on claims and/or results of personal health assessment • Complete Disease Management, Case Management, OR Maternity intake assessment • Complete 16 weeks survey (28 weeks for maternity) • 3 active counseling calls from Personal Health Coach during program • Completion of one goal during program • Covers diseases like diabetes and asthma

  6. Health Reimbursement Account • You and your spouse must • Complete the personal health assessment (PHA) • Complete three wellness courses • You will receive • $200 the first program year (7/1/2007 to 12/15/2008) • $250 the second program year (1/1/2009 to 12/15/2009) • Health Reimbursement Account – Pre-tax account • Co-pays & co-insurance • Deductibles • Over-the-counter medicines • Funds can roll over if not spent

  8. H&PM Wellness Activity:$$ Shared Savings $$ • Savings from program are split 50/50 between MPS and MTEA • Savings from July 1, 2007 to June 30, 2008 shared with current employees who took the PHA by December 15, 2007. (Spouse must also have completed the PHA.) • You will continue to share savings if: • You and spouse complete the PHA • You complete a voluntary program • You are a current employee • Funds are deposited in an HRA.
