  2. Application Process • Application sent to applicant • Application completed and, when necessary, professional verification received • Application mailed or faxed to LYNX • Reviewed for completeness and entered into database

  3. Application Process • TD is approved or denied based on flow chart • ADA & Medicaid are referred to Functional Assessment or Approved based on self certification • Applicant schedules Functional Assessment

  4. Application Process • Applicant transported to Functional Assessment • Cognitive test performed • Physical test performed • Application transported from Functional Assessment • Functional Assessment emailed to LYNX

  5. Application Process • Eligibility determination made by LYNX staff • Applicant notified of approval or denial • Denied applicants referred to Fixed Route Travel Training • Application and all correspondence, documentation, functional assessment, and Medicaid verification filed by date

  6. Fixed Route Travel Training • Requesting bus information • Planning a trip • Learning to read bus schedules • Boarding and exiting the bus • Fare payment and pass usage • Pedestrian safety • Emergency procedures • Stranger awareness • Appropriate behavior • How to use the wheelchair lift/ramp
