  2. Bridging Communities & clinics • Evidence-based pilot initiated in 2012 • Integration of preventive health screenings and local health clinics with community events • Cultural and linguistic considerations • Cost-efficient, accurate screenings • Training of health sciences students as CHWs

  3. PILOT Program OVERVIEW • Clinical referral network • 16 clinics (UDOH, FQHC, private, non-profit) • 5 counties (Weber, SL, UT, Summit, Grand) • Satellite sites • 5 dedicated locations; screenings, classes • Community partners • 25 CBOs, FBOs, schools, community centers

  4. CHW roles • Preventive Screenings • Glucose, cholesterol, blood pressure, BMI • Clinical Referrals • Primary care provider referrals • Follow-up by telephone, email • Community Guidance • Language assistance • Assistance accessing services
