  2. Contact with TA Provider • Most will have the same TA Provider as in Year One • You will maintain regular contact with TA Provider as you did in Year One

  3. Site Visits for Outreach Grantees • Opportunities for grantees to attend a peer-learning symposium in Atlanta • Meet with other grantees with similar programs • Meeting will serve as substitute for an on-site visit by your TA Provider

  4. Site Visits for Outreach Grantees • 1 ½ day meetings • Agenda will include: • Time for grantees to present programs to one another • Expert speaker in the program area • Sustainability training

  5. Site Visits for Outreach Grantees • November 27 – 28, 2007 • Physical Fitness and Nutrition • Health Education and Health Screenings

  6. Site Visits for Outreach Grantees • February 20 -21, 2008 • Case Management • Promotoras Programs

  7. Site Visits for Outreach Grantees • March 18 - 19, 2008 • Oral Health • Mental Health, Behavioral Health, Substance Abuse

  8. Site Visits for Outreach Grantees • Cost for one participant per grantee to be covered by GPHC • May send additional participants

  9. Site Visits for Outreach Grantees • Some Outreach grantees will receive on-site visits if there are no appropriate programs of interest • Your TA Provider will provide further information as specifics are developed

  10. Site Visits for Network Development Grantees • Most will receive a second on-site visit from your TA Provider • May attend Peer Learning Symposiums, if appropriate
