POPP – The Big Picture • Funded from ODPM Central Government • Department of Health £60m • 19 Round One Pilot Projects • 3 in South West of England – Dorset, Somerset, Poole • Approaches vary dependent on local priority • Initial 2 year funding • Years 3 & 4 funded by achievement of year 1 & 2 • Positive interventions identified by local older people • Older person ‘led’

  3. The Local Context • Dorset Population c. 400,000 • Largest percentage of older people in the country – 29.3% aged 60+ in Dorset compared to 20.9% nationally • Christchurch - 36.1% of the population aged 60+ • Dorset POPP Focus aged 50+

Aim of Dorset POPP • To work with older people to develop supportive neighbourhoods where older people can live and receive appropriate, timely help and support to stay within local communities for as long as they wish • Over a two year period POPP will support and develop positive interventions and preventative actions identified by local people. This will ensure cost savings on the provision of acute services, and on the take up of hospital bed spaces, which in turn will be clawed back into supporting a sustained POPP into years 3 and 4

  5. Dorset POPP Partnership • The project is a partnership between • Older people • Voluntary organisations • District Councils • County Council • NHS and PCT • With co-operation and commitment on the Steering Group from Police, DFRS, Housing providers....

Anticipated Gains for Older People • An opportunity to ‘Challenge and Change’ • Greater Confidence in working effectively in partnership with providers of health & Social Care • Increasing awareness of services and support available within the community • Increased access to local information, resources, and transport • Social activities, befriending, shopping and practical support • Reduced isolation and increased sense of community safety

Anticipated Gains for Health Service • POPP will enable greater choice for Practice Based Commissioning – identifying local service need and provision • Addressing the issues of spending on acute services • Reduction in the uptake of hospital bed spaces by addressing the issues of delayed discharge

POPP Targets • Save 1188 Acute hospital bed days • Help to prevent 792 A & E attendances • Help to prevent 24 care home admissions • Help to prevent 66 home care packages Community Development Programme to work with statutory organisations to support targets on: Expert patient programme, long-term conditions, mental health promotion, and general health promotion

  9. Dorset POPP Structure • Project Manager • 4 Community Development Workers • 1 Performance Analyst • 33 Neighbourhood Cluster • 2 Local ‘Way-Finders’ per Cluster (total 66) • 1 Local ‘Leader’ per Cluster (total 33) • 15 Local Evaluators

Local Leaders • Local Older People aged 50+ • Receive training in Leadership skills and support to help bring about change in each of the neighbourhoods • Infiltrate local meetings and partnerships such as Parish Council, Town Council, PCT PPI Forums, committees • Influence local service providers and develop and shape services based on the needs of older people • Use local knowledge and experience to identify existing facilities for establishment of community resource centres

Local Way-Finders • Local Older People aged 50+ • To provide advocacy, information and sign-posting • Supported by the local Leaders • Raise awareness of services available • Make information accessible to local older people in their communities • Make Links to preventative, health promotion, and support services • Identify support and help within communities for older people being discharged from hospital and other institutional care providers

Local Evaluators • To measure change as it occurs during the life of the project • To use emerging evidence to influence the way services are provided by local councils and health • Working alongside Local Leaders and Way-Finders • Within and across neighbourhood clusters • Evaluate how services are working and what impact they are having on the lives of older people • Evaluate the success of the partnership, the Board and the structure of Dorset POPP

Practical Changes • Working with older people to develop ideas on services that will help older people to live independently and will promote positive initiatives for good health and social activity: • Safer property – Handy van and staying put schemes • Physical environment – assistive technology, ‘sloppy slipper’ falls reduction • Intellectual and spiritual stimulation – Arts Reach, Book Clubs etc. • Companionship – Lunch Clubs, befriending, coffee mornings • Good Health – Exercise uptake, Healthy eating • Older Peoples Services Mapping undertaken

Progress to Date……. • £2.4 million awarded for 2006/07 - 2007/08 • Going Live Launch April 24th – 1764 contacts • Project Manager in post 1st May 2006 • Contracts awarded for Leadership/Evaluator prog • Board and Steering Group Structures assessed • 260 further information requests • 4 CDW’s interviewed and appointed • Publicity - Way-finders, Leaders, Evaluators • Appraisal System established for applications for £ • Contact established with key local partners inc. OPF’s, Age Concern, Help and Care, Help The Aged, Volunteer Bureaux, Community Link Officers, etc. • Engagement in other activities: Contracting/Commissioning etc
