  6. Council Program Director • Appointed by the Grand Knight • Help plan & schedule council programs • Help project chairmen • Insure preparations for projects are completed in a timely manner • Keep records & make sure forms are filed ASAP but never later than the due date

  16. Columbian Award Forms Submit: • Service Program Personnel Report form (#365) submitted by August 1. • Annual Survey of Fraternal Activityform(#1728)submitted by Jan. 31 • Submit form SP-7 by June 30 **Forms Must Reach Supreme** By Due Dates

  17. Michigan Achievement Award and Michigan Form #MI-01 • Submit all MI-01 reports on time. • Complete a minimum of 1 activity in each of category; Church, Community, Council, Family, Youth, and Culture of Life specified on the MI-01 form.

MI-01 • The new MI 01 form will be on the State web site as a web based form which can be completed on line. • To qualify for the Michigan Achievement award it is necessary to complete one program in each of the six categories each quarter. • Enter the credit in the appropriate cell after completing the program. This will help in forecasting and planning necessary credits to qualify. • Programs options will be available from a drop down tab.

MI-01 • Distribution will be done automatically and District Deputies, Diocesan Program Directors and the State Program Director will have access to oversee completion. • Projection of Michigan Achievement for recognition can be accomplished if councils enter credits as the programs are completed rather than waiting until the end of the fourth quarter.

  21. MI-01 • Programs unlisted may be listed by adding a line to the form. • If necessary a hard copy may be printed, completed by hand and given to the District Deputy who will then enter data on the web based form.

The Service Program Awards • Identify a special or unique project your Council is planning or has held in the past • Review Leadership Guide Program section and web site for detailed information. • Appoint one individual to collect all the records of the program from the very beginning to the final stages

Collect: • Correspondence – Actual letters sent & received • Photographs • Copies of all press releases sent out and all published media: bulletin, newsletter, diocesan newspapers, local newspapers • Record of hours & the number of members, family members and guests participating • Financial summary including budget, actual expenses & funds raised • Acknowledge and Thank – all volunteers, suppliers, donors, media and other parties that had a hand in the success of the project
