Our project addresses the critical issue of medication non-adherence among seniors, which accounts for 30% of hospital readmissions and costs the healthcare system over $15.2 billion annually. With the Pre-package solution, patients taking multiple prescriptions receive organized dosages in portable bags, enhanced by QR-code accessibility for educational support. This solution enables easier medication management, promotes understanding of prescriptions, and ensures patients take the correct dosage at the right time. Streamlined pharmacy collaboration ensures timely delivery of medication to patients at home.

Problem • Beginning in 2013, hospitals will be bearing part of the costs of readmissions. • 30% of hospital admissions for seniors over 65 are due to medication non-adherence. • lack of adherence costs the healthcare system over $15.2 billion annually • 75% of the over-65 population takes at least one Rx (and most taking 2-4 per day), these complications can increase adherence problems • Our project addresses the following reasons for non-adherence. • Patient education: Studies have shown up to 60% of patients misunderstood their prescription information after leaving the doctor. • Multiple medications: Studies have also shown that higher frequencies of doses increase adherence problems exponentially.
How Can It Help? • Pre-package is a combination between medication management and modern technology that helps patients taking multiple medications to take the correct dose at the correct time. • Information: The QR-code provides an educational aspect to understand why taking the medicine is important. • Portability: The small bags can be torn apart independently or by day, allowing patients to carry medication anywhere. • Educated control: With the pharmacies packing the correct medication, managing multiple medications is much easier for patients.
The Process • Prescription is electronically sent to Pharmacy • Physician asks patient to join the pre-package solution • The pharmacy packs the first week of prescription • The patient picks up the first week’s worth • The pharmacy packs the medicine and sends it to the patient home every 2-4 weeks
Patient Education • The QR-code that can be accessed from any smart phone provides educational information with a variety of aspects. • Written instructions can be read • Visual images can be seen • Audio option can be heard Try it yourself.
Instructions • Open a QR scanner app on your smart phone • Focus in on the QR-code and wait for the website to show up • Read the Information, see the medication pictures, listen to audio option
