As You Receive WRITING TEST MATERIALS from the Test Administrators, do the following: __1. Verify that all distributed Writing Folders have been returned. Notify the district coordinator immediately if any Writing Folders are missing. __2. Remove stray Planning Sheets from Writing Folders. __3. Verify that you have collected all required administration information. Make a copy for your files. (Page 17 of the manual) __4. Verify that you have collected all Security Checklists. Make a copy for your files. __5. Verify that your Security Logs have been completed correctly. Make a copy for your files. __6. Verify that each student has completed the Student Name and District/School in the upper right corner of the student grid sheet, even if a label has been affixed in the lower left corner. __7. Check for and erase stray marks ONLY in the student label area and the security number area of the student grid sheets.
As You Receive WRITING TEST MATERIALS from the Test Administrators, do the following: • __8. If a Writing Folder is TO BE SCORED, verify that the DNS bubble has not been gridded. If a DNS bubble has been gridded by mistake, erase the DNS bubble and grid the UNDO bubble. • __9. If a Writing Folder is NOT TO BE SCORED, verify that the DNS bubble has been gridded. • __10. Organize your Writing Folders into stacks as follows: • (Page 31-32 of the manual) • Your School’s Answer Documents • Grade 4 Writing Folders • Grade 8 Writing Folders • Grade 10 Writing Folders • Home Education Programby grade level (49-9998) if applicable • Grade 4 Writing Folders • Grade 8 Writing Folders • Grade 10 Writing Folders

Preparing Your School’s TO BE SCORED Answer Documents For Return • ___11. Complete a separate • Document Count Form for each grade level. (page 38) • DO NOT combine Writing Folders for more than one grade level under the same form. • Verify that you have included all Writing Folders from make-up sessions before you complete the forms. • The Count Form is scannable and must not be photocopied. • Place each completed Count Form on top of the first stack of corresponding TO BE SCORED Writing Folders.
Complete the ANSWER DOCUMENT RETURN SUMMARY • __12. Complete the information on the Answer Document RETURN SUMMARY as instructed below. (page 39) • Complete only one ANSWER DOCUMENT RETURN SUMMARY for students enrolled in your school. (There are separate forms for Home Education, McKay Scholarship, and Osceola Virtual School) • Transfer the information from each DOCUMENT COUNT FORM to the ANSWER DOCUMENT RETURN SUMMARY. Each completed DOCUMENT COUNT FORM must have an entry on the ANSWER DOCUMENT RETURN SUMMARY. • Verify that the number of your school’s TO BE SCORED Writing Folders returned and the number of folders indicated on the DOCUMENT COUNT FORMS match the entries on the ANSWER DOCUMENT RETURN SUMMARY. • Maintain a copy of the completed ANSWER DOCUMENT RETURN SUMMARY for your records.
Complete the ANSWER DOCUMENT RETURN SUMMARY • __13. Each school will receive an Answer Document Return Summary with the following information preidentified: • District Name and Number • School Name and Number • Grade Level and Subject Tested • Complete the preidentified Answer Document Return Summary Form which have been provided. • A blank Answer Document • Return Summary Form • may be found at www.ctb.com/fcat or the REA website
Complete Paper Bands __14. Documents for each grade level must be banded separately. You may use more than one paper band per grade level, if necessary. Follow these steps for each paper band that you use: • Grid the appropriate grade level/document type. • Write the school name and district/school number at the bottom of the Paper Band. • Count the number of TO BE SCORED Writing Folders that will be banded in each paper band. Write that number under Number of TO BE SCORED Documents Contained in This Paper Band. • Under Number of Banded Stacks per Document Count Form, number the paper bands 1 of n, 2 of n, etc., where n is the school’s total number of banded stacks per Document Count Form. • Place a paper band around each stack of TO BE SCORED Writing Folders. The Document Count Form must be under the paper band on top of the stack labeled 1 of n. • Seal the Paper Band and be sure that it securely holds the Writing Folders. Do not use staples, paper clips, or tape.
Preparing Home Education/Private McKay Scholarship Programs and Osceola Virtual School TO BE SCORED Documents For Return Process Writing Folders for all Home Education Program (49-9998 ) separately from your school’s TO BE SCORED Writing Folders. Process Writing Folders for all Osceola Virtual School (49-7001) separately from your school’s TO BE SCORED Writing Folders. Process Writing Folders for all Private McKay Scholarship Program students (49-3518) ENROLLED IN PRIVATE SCHOOLS separately from your school’s TO BE SCORED Writing Folders. Scores for these students will be returned to the District Office for distribution, but results will NOT be aggregated with district or school data.
Completing the Document Count Forms for Home Education/Private McKay Scholarship And Osceola Virtual School • ___15. Complete the DOCUMENT COUNT FORM by following the directions on the form. (Page 41-45 in the manual) • A separate Count Form must be completed for each grade level and for each program. • Do not combine grade levels or Programs under the same DOCUMENT COUNT FORM. • Write the appropriate Program name in the School Name space. • In the space labeled District/Program Number, write the appropriate district number and Program Number.
Prepare Florida K-8 Virtual School Program TO BE SCORED Writing Folders For Return ___16. Place all TO BE SCORED Writing Folders for Connections Academy students in the envelope marked Connections Academy and seal it. ___17. Place all TO BE SCORED Writing Folders for Florida Virtual Academy students in the envelope marked Florida Virtual Academy and seal it.
Packaging Writing Test Materials You will be preparing the following types of boxes for return to the District Coordinator: All TO BE SCORED Writing Folders – RED Labels Due February 18, 2009 All NOT TO BE SCORED Materials – YELLOW Labels Due February 24, 2009 All Large Print and Braille Materials (if applicable) – WHITE Labels Due February 18, 2009 District Coordinator ONLY Box – Create your own label Due by March 2, 2009 *It is your responsibility to package all materials correctly for processing and score reporting. Mispackaged materials will delay reporting of student results.
Packaging The Red Label Boxes • Banded stacks containing DOCUMENT COUNT FORMS, and your school’s “TO BE SCORED” Writing Folders by grade level • Answer Document Return Summary • Banded stacks containing Document Count Forms and “TO BE SCORED” Writing Folders by grade level for Home Education, Private McKay Scholarship Program, and Osceola Virtual School • 4. Answer Document Return Summary • Envelopes containing “TO BE SCORED” Virtual School Program Writing Folders for Connections Academy • Envelopes containing “TO BE SCORED” Virtual School Program Writing Folders for Virtual Academy • Return the box(es) to the District Coordinator by February 18, 2009
Labeling The Boxes Place the CORRECT COLORED LABEL on the top of each box.
Preparing LARGE PRINT and BRAILLE Materials For Return • Complete the following steps after testing: • 1. Verify that you have collected all required administration information (see page 17). Make a copy for your files. Notify the district coordinator immediately if any secure test materials are missing. • 2. Verify that your Security Log has been completed correctly. Make a copy for your files. • 3. Remove stray planning sheets from the test materials. • 4. Check each Large Print and Braille Materials Return Envelope and verify that each envelope contains documents for one student only. Materials for one student may be placed in more than one envelope. • 5. Verify that each student grid sheet has been completed correctly or has a student label affixed.
Preparing LARGE PRINT and BRAILLE Materials For Return • Complete the following steps after testing: • 6. Ensure that the student name and district/school number appear in the appropriate locations. See page 61 • 7. If a Writing Folder is TO BE SCORED, verify that the DNS bubble has NOT been gridded. If the DNS bubble has been gridded by mistake, erase the DNS bubble and grid the UNDO bubble. If a Writing Folder is NOT TO BE SCORED, verify that the DNS bubble has been gridded. • 8. Group each student’s materials for return to the district coordinator. Place the following materials for each student tested in a separate LARGE PRINT AND BRAILLE Materials RETURN ENVELOPE. Make sure that each student’s envelope contains the following materials:
Preparing LARGE PRINT and BRAILLE Materials For Return (continued) • LARGE PRINT • Completed Document Count Form • Large Print Writing Folder • BRAILLE • Completed Document Count Form • Braille Final Writing containing student responses • Braille Writing Folder • Regular Print Writing Folder Note: Although you may place documents for more than one SUBJECT in the same envelope, do not place materials for more than one STUDENT in the same envelope. DO NOT place NOT TO BE SCORED materials in a Large Print and Braille Materials Return Envelope.
Complete the LARGE PRINT and BRAILLE Materials Return Envelope • 9. Verify the completed information on the outside of each student envelope and seal the envelope. • 10. Complete the Large Print and Braille Answer Document Return Summary according to the directions on the form, and place it on top of the Large Print and Braille Materials Return Envelopes in the first box of Large Print and Braille materials. • 11. Place the appropriate Large Print and Braille test materials into the original boxes in which they arrived, according to the diagram on page 66. • Place a WHITE label anywhere on top of each box. Do not copy labels. • Do not put Large Print or Braille materials in the same boxes as regular TO BE SCORED • or NOT TO BE SCORED materials. Mispackaged materials will delay reporting of student results.
Packaging Large Print and Braille Materials - White Label • 1. “NOT TO BE SCORED” Writing Folders • with DNS bubble gridded • Unused Writing Folders • BRAILLE Notes • Document Count Form • LARGE PRINT • Writing Folder goes inside the preprinted envelope. (One for each student) Place the envelope into the box in which • they arrived. • 4. Document Count Form • 5. BRAILLE • Braille Final Writing Sheets containing student responses • Braille Writing Folder • Regular Print Writing Folder • All go inside the preprinted envelope. (One for each student) Place the envelope into the box in which they arrived.
Packaging Large Print and Braille Materials - White Label • 6. Large Print and Braille Answer Document Return Summary • Seal the boxes. • Place a white label on the top of each box, according to the diagram on page 67. • Use the space on the labels marked BOX_ OF _ to number the boxes 1 of n, 2 of n, etc., where n is the school’s total number of boxes with white labels. • Return the box(es) to the District Coordinator by February 18, 2009
Preparing Your School’s Not To Be Scored & District Coordinator Test Materials For Return • All NOT TO BE SCORED Writing Folders • unused non-preidentified and preidentified Writing Folders • defective folders • folders with the DNS bubble gridded • District Coordinator Only Box • Test Administration Manual – Store at your school until FCAT Writing test results are received for all students • Planning Sheets (Used and Unused) – Place in District Coordinator Box • Security Log – Place in District Coordinator Box • Required Administration Information – Place in District Coordinator Box
Packaging NOT TO BE SCORED Materials Yellow Labels • 1. Place all NOT TO BE SCORED materials in boxes This would include: • Unused non-preidentified Writing Folders • Defective folders • Preidentified Writing Folders • Folders with the DNS bubble gridded 2. Seal the box(es) • Place the YELLOW LABEL on the top of the box • Use the space on the labels marked BOX _ OF _ to number the boxes 1 of n, 2 of n,, etc., where n is the school’s total number of boxes with yellow labels 5. Return the box(es) to the District Coordinator by February 23, 2009.
Packaging District Coordinator Only Box • 1. Place all non-secure materials in the box. This would include: • Planning Sheets (used and unused) • Original Security Checklists • Original FCAT Administration and Security Agreement (ONLY Test Coordinator and Test Administrator copy) • Original FCAT Administration Record form • Original Records of Attendance • Original FCAT Security Logs • Original Seating Charts • Original Student Pre-ID Roster • Unused Student Pre-ID Labels • Unused non-secure Large Print and Braille materials 2. Sealing The Boxes Points To Remember 1. Make sure the packing tape does not cover the label. 2. Labels are scannable and should not be photocopied. 3. Do not place labels over the meeting point of the two flaps.
