Project List • Illinois Department of Human Rights, Employment Complainant Information Sheet Description: This form is used to file a claim of discrimination in an employment situation in Illinois. • Illinois Department of Human Rights, Public Accommodations Complainant Information Sheet Description: This form is used to file a claim of discrimination in the enjoyment of facilities and services by any place of public accommodation, such as a business, recreation, lodging, entertainment, or transportation facility that is open to the public in Illinois. • Illinois Department of Human Rights, Housing/Real Estate Transactions Complainant Information Sheet Description: This form is used to file a claim of discrimination in the sale or rental of residential or commercial property in Illinois. • Idaho Nez Perce Tribal Court, Civil Complaint Description: These forms are used to file a civil complaint with the Nez Perce Tribal Court in Utah. These forms and instructions will be combined to make one HotDocs template with one coordinating A2J Guided Interview. • Pennsylvania Petition for Expungement & Order Pursuant to Pa.R.Crim.P. 490 (In Summary Cases) & 790 (In Court Cases) Description: Pa forms are used to expunge a criminal record in cases where the charges were dismissed (490) or the there was a conviction (790). The Petitions and Orders under both parts of the statute are very similar and will be combined to make one HotDocs template with one coordinating A2J Guided Interview. • Ohio Motion for Expungement, Affidavit & Order Pursuant to P.C. 2953.32 Description: The forms below are used to seal a criminal record in cases where there was a conviction or there was not a conviction. The Motions, Affidavit and Orders for both cases are very similar and will be combined to make one HotDocs Template with one coordinated A2J Guided Interview.
