ELIGIBILITY GUIDELINES • Demonstrated experience in schools • Capacity to work PreK – Grade 12 • Submit only one concise program model • Provide evidence of Fed Tax ID/SS# • Be aware of NPS fingerprinting, insurance and contracting procedures
REQUIREMENTS • Have a mailing address • Have a phone or fax • Have an email address • Be aware that any potential contracts that may exceed $36,000 will be required to submit paperwork to the PQS (Pre-Qualification Solicitation) at NPS
RESPONSIBILITIES • Meet regularly together to share ideas • Implement NPS arts evaluation tool • Develop/Maintain accurate records • Be flexible • Be aware that participation provides no guarantee of school requests
APPLICATION INSTRUCTIONS • Submitted electronically; No faxes • Assurances/Signatures can be mailed or scanned • First-round submissions must meet all deadlines; revolving submissions after • Identify acronyms/abbreviations in ap • No brochures, publicity materials • Keep a copy for your files

INELIGIBLE COMPONENTS • Expenses outside the service delivery time • Out of state travel • Admin/Indirect costs • Programming that replaces NPS services • Activities un-related to Arts Learning Plans • Programs outside NPS schools • Claims of intellectual/creative property • Variable fees
