Create a Incident Safety PlanState Practical Skill Exam • Using the “Structure Fire Safety Report” complete the 3 page form and submit for evaluation from the information in this presentation • Forms need to be filled out completely, and clearly printed. • This skill will be completed in front of the Evaluator in the classroom. • Each slide will be shown for up to 2 minutes each

Create a Incident Safety Plan • Students will fill out forms during presentation. • Students will have 5 minutes at end of presentation to finish forms. • Students may use a blank sheet of paper to take notes during the presentation. • NO DISCUSSION during this exam • Students must complete this skill alone

Units Dispatched • Engines, Truck, and Rescue Companies are staffed with either 3 or 4 personnel which includes an officer, engineer, and firefighters. • The squad is an ALS unit with one medic • An accountability system is used by the fire department. Helmet tags are used for personnel and vehicle.

  6. Fire Department Dispatch • Incident # 10-255 • Date: use exam date • 9915 Mount Vernon Road • 1315 hours – dispatch • Residential fire

  8. Units Dispatched • Engine 1, crew of four • Engine 2, crew of four • Truck 4, crew of four • Engine 3, crew of three • Rescue 1, crew of four • Squad 2, crew of two • Battalion Chief 2 & 4 • ISO Unit #205

Fire Department Dispatch • 1319 hours – first unit on scene (Eng. 2) • Confirms working two story wood frame residential fire • 1321 hours - BC 2 arrives and takes command • IC Post setup in the street on side A • Engine 1 & 3 arrive on scene • 1322 hours – ISO Unit #205 arrives & reports to Incident Command post (ICP)

  10. Arriving Units • 1322 hrs. Truck 4 arrives • 17 personnel on the scene • 1323 hrs Rescue 1 arrives and assigned RIT on A side of structure • Garage is fully involved • Water supply established by Engine 1 hitting the hydrant at the corner

  12. Arriving Units • IC announce offensive attack in place. • Engine 2 crew performing a primary search with a 1 ¾” hoseline on first floor. They enter the structure on the A side • Engine 1 is assigned to deploy a 1 ¾” line around to the B side of the structure for fire attack from B-C corner • 1327 hrs. Engine 3 crew arrives and are assigned to second floor for search with 1 ¾” line, they enter on the A side

  14. Scene Action • The fire was located on the B end of the structure, involving the garage and extends to the second floor and into the attic. • 1330 hrs. Truck 4 is placed in front of structure and crew is lifted above the structure to monitor operations

  16. Scene Action • Staging is setup in front of Truck 4 • Interior crews advise there is no basement • 1331 hrs BC 4 arrives and assigned Operation Chief • 1332 hrs Engine 3 reports heavy fire on second floor • Ventilation took place by breaking out windows on first and second floors

  19. Incident Information • Squad 2 is assigned rehab and setup across the street • 26 personnel on the scene • For this practical, PPE and SCBA are being used satisfactory • CO levels being monitored by Truck 4’s crew

  20. Incident Information • At 1335 Engine 2 crew reports all clear on first floor • At 1335 hrs IC orders crews to evacuate structure. • IC orders operations to defensive mode. • Master streams deployed to side A and D

  21. Utilities Information • Utilities on at time of fire, they enter the structure from B side • Utilities include electric, natural gas • Breaker panel located rear wall in garage • IC calls dispatch to contact utilities for disconnection of services

  22. Incident Information • Structure has engineered floor joists • Home built 4 years old • Exposures on B and D sides about 20 to 25 feet away. • IC orders Operations to send crews to rehab after their second air bottle.

  23. Incident Information • 1336 hrs IC ask for PAR • 1345 hrs fire knock down on B side • 1346 hrs. Crews continue operations in defensive mode • 1352 hrs IC marks fire under control

  24. Incident Information • 1353 IC orders crews to start overhaul operations • IC orders Truck 4’s crew to continue to monitor CO level inside structure during overhaul • All units remains in SCBA until levels reduce to under 35 ppm • 1359 hrs IC calls for PAR

  25. TIME to FINISH • You have 5 minutes from now to complete the forms and submit them to the Evaluator. • On an actual incident the reports would be submitted to the IC and inserted into the Incident Action Plan.

  26. TIME UP! • You will be asked to hand the Evaluator your forms. • Time may be given after all forms have been turned in to discuss the plans and any issues with the scene. This will be with the Instructor while the Evaluator is evaluating the forms.
