  3. PURPOSE To aid the transition process for separating and retiring service members who intend to file a claim for VA disability benefits no earlier than 180 days and no later than 60 days prior to separation of the service member.

  4. GOAL The Goal of the Veterans Administration Benefits Delivery at Discharge Program is to prepare and provide a formal rating decision of your claim as near to the time of discharge as possible.

  5. APPLICATION PROCESS • Attend Fort Lee’s Transition Assistance Program Workshop • Complete only Part I of Retirement Physical at KAHC • Request certify copy of medical record • Turn in application (VA Form 21-526) to VA Representative Note: Application from can only be turn in to VA Representative during schedule time at Fort Lee

  6. DOCUMENTATION REQUIREMENTS • Part I Physical Exam • Certified copy of medical records • Children birth certificates • Marriage Certificates (if applicable) • Divorce Decrees (if applicable) • VA Form 21-526

  7. MEDICAL APPOINTMENTS • The Department of Veteran’s Affairs through the QTC Examination services will conduct Part II of the retirement physical using not only VA’s examination protocols but also Service-recommended laboratory tests and screenings, when applicable.

Soldier attends TAP workshop VA Rep delivers claim form and medical records to Regional Office (Claims Dept) Turn in application to Retirement Service Office during final out appointment No Does soldier meet BDD timelines? Soldier do not meet BDD Timelines? QTC request Part I Exam Yes (1) Soldier completes draft VA Claim Form 21-526 (2) Schedules Part I Physical Exam (3) Soldier requests a Copy of Medical Records QTC notifies claimant 48 hours prior to scheduled Part II exam appointments Claimant attends Appointments. Physical Exams office delivers Part I exam Exam, reports and diagnostics received by QTC. VA Rep reviews and collects VA Claim Form 21-526 and certified medical records A Completed reports Uploaded to Exam Track. VA downloads reports From Exam Track VA processes claim. Part 1 Physical Exam Instructions *Call Tricare to book Part 1 Retirement Physical @ 866-645-4584. *If no appointment available thru the Tricare appointment line, contact the Physical Exam Office between 0900-1500 hrs @ (804) 734-9231/9057. *Make sure you tell the clerk you are a participant of the BBD program *ONLY make appointment for Part 1 Retirement Physical. *All Part 1 Appointments start promptly at 0630 hrs at the Pharmacy waiting area. *Arrive 10 minutes prior to the appointment time. *Late arrival appointments will be terminated and appointment must be reschedule after 0900 hrs through the Active Duty Clinic. *Patient must be in P.T. uniform with identification card and medical records * Patient must remove their contact lenses 24 hours prior to their appointment and have their glasses with them. * Patient must also fast beginning at 2030 hrs the night before their Part 1 physical NO FOOD, CANDY, COFFEE, GUM OR ALCOHOL, NOTHIN BUT WATER. Please drink plenty of water so you can provide a urine sample.
