SPRING INDEPENDENT SCHOOL DISTRICT Lewis Elementary School 3230 Spears Road * Houston, TX 77067 * Phone (832) 764-8720 * Fax (281) 440-8676 Dr. Barbara Jones, Principal Mrs. Rebecca Perez-Rangel, Assistant Principal • WELCOME TO LEWIS ELEMENTARY SCHOOL • To register a new student, you will need the following: • Original Birth Certificate • Shot/Immunization Records • Social Security Card • Parent/Guardian Driver’s License or TX ID • Proof of Residency (light, water, or gas bill, or lease/mortgage contract ONLY) • If registering a child for Pre-K, we must have proof of income (recent pay stub or W-2 form.) • If you are registering a new student from another school, you will need • the following: • Last School Year’s Report Card along with any test students have taken. • Any test that students have taken in last school year. • Withdrawal paperwork from previous school, if applicable. • If you are missing any of the above items, we will be unable to process • your child’s registration paperwork. Please return when you have all of • the items needed. All COMPLETED paperwork needs to be turned into • the Registrar. • Thank You • Lewis Elementary Registrar
