  9. Page 1: Important Points Where do students depart from and return to? Hyatts Middle School, Liberty Middle School & Olentangy Liberty High School depart from the West Lot of Olentangy Liberty High School

  10. Page 1: Important Points Where do students depart from and return to? Orange HS, Orange MS, Shanahan MS, Berkshire MS and Olentangy HS depart from the North Lot of Olentangy HS, in front of the student services entrance

  14. Page 1: Important Points EMERGENCY PROCEEDURES - staff member notified - we call parent - parent may be asked to pick up student - student may be transported to Mary Rutan Hospital or - We bring injured student home on bus

  17. Page 2: Important Points Ski Club Rules Food – okay but no glass bottles and no milk or milk products. Must clean up – throw away in trash bags (see rule 7)

  26. Know Your Responsibility Code 1. Always stay in control, and be able to stop or avoid other people or objects.

  27. Know Your Responsibility Code 2. People ahead of you have the right of way. It is your responsibility to avoid them.

  28. Know Your Responsibility Code 3. You must not stop where you obstruct a trail, or are not visible from above. If you fall in an obstructed view area, try to get someone to assist you by standing above you in an area that is not obstructed.

  29. Know Your Responsibility Code 4. Whenever starting downhill or merging into a trail, look uphill and yield to others.

  30. Know Your Responsibility Code 7. Prior to using any lift, you must have knowledge and ability to load, ride and unload safely.

  32. Filling out the Application Step 1: Fill out Page 5. - Make sure to include your name. - Make sure to circle the school you are attending. • Circle drop-off / pick-up point • Tell us about your plans and experience - Read and Sign the agreement of participation

  33. Filling out the Application Step 1b: Fill out the Emergency Medical Authorization Form and the Mary Rutan Hospital Form. (these are the last two forms in your packet)

  34. Filling out the Application Step 1c: Complete and have Notarized: The Mary Rutan Hospital Form This is necessary, just in case

  35. Send it in! Deadline Nov. 18th Return the application and the two medical forms to school along with a check made payable to Olentangy Local Schools or Olentangy Ski Club Check amount is $72.00 which covers the cost of transportation. (Lakefront Buses)

  36. Step 2 Go on-line and buy your pass

  37. Step 3 Send Mad River Mountain your picture • Using your smart phone or a camera, take a head shot picture. • Send the picture to: pass@skimadriver.com Name & Birthdate in subject heading
