Guidelines for Clinical RotationsDocumentation Requirements Review Clinical Rotation Request Packet Submit clinical rotation request form – one clinical rotation request per form Two weeks before clinical rotation, submit: 1. Education Requirement Grid 2. Student Information Sheet 3. Student Signature Form 4. HIPAA Training Signature Form 5. Course objectives/syllabus * (new)
Requirements • Arrive on time. If you are coming in at change of shift, have students ready for report • Wear ID badge on upper torso • Clean, neat, professional appearance • No artificial fingernails • Long hairstyles should be worn with hair pulled back • Accudose access will not be granted to instructors or students
Instructor Responsibilities • Orient students to unit • Vital signs • Monitors • IV pumps • Co-sign all flowsheet and nursing note documentation • Medications administered by students must be documented under instructor’s EPIC log-in
Observation opportunities • Based on availability • ALL requests must go through either Denise or Margaret • Comer request contact Denise LeNeave, BSN, RN, CPN • Mitchell and Women’s Care Center request contact Margaret Gleason, BSN, RN, OCN
Badge Distribution • Instructor to pick up badges one or two days prior to start of rotation (C135) • Each student receives a designated badge • Badges are returned at the end of rotation ($15 charge for lost badges) • Badges cannot be submitted until all required documentation is received
Graduate Rotation Placement Procedure • Graduate student contacts Central Educator with rotation request • Request is sent to APN Committee members for review • Once preceptor is established, student must submit required paperwork to Central Educator and will obtain a student badge
