  2. Vroege Non-motor symptomen • Vroege “markers” bij PD: • Positieve familie-anamnese • Reukafwijkingen • Slaapstoornissen • Depressie • Vroege neuropsychologische symptomen • Autonome dysfunctie • Visusproblemen • SPECT en PET-scan afwijkingen • Transcraniële echo GeriatrieAvond Westfriesland

Vroege Non-motor symptomen • Reukafwijkingen (hyposmie/anosmie) • Aanwezig jaren voor motorische problemen (4-6 jaar) • Uiteindelijk in ± 80% • Onderscheiden van geuren gestoord • Dopaminerge medicatie heeft een negatief effect op reuk • Geen relatie met hoeveelheid neuronen verlies (MPTP geen reukstoornis) • Verlies neuronen en tractus olfactorius en LB in tractus GeriatrieAvond Westfriesland

  4. Vroege Non-motor symptomen • Reukonderzoek in te zetten bij vroegdiagnostiek • Vermindering reuk→ SPECT: afwijkingen in striatum • Onveranderde reuk→SPECT: geen verandering striatum • PSP, MSA, CBD: geen reukafwijkingen GeriatrieAvond Westfriesland

  5. Vroege Non-motor symptomen • Reukafwijkingen ook bij MCI, dementie van het Alzheimertype • Meerdere markers nodig bij diagnostiek samen met SPECT GeriatrieAvond Westfriesland

Vroege Non-motor symptomen • Depressie en Ahedonie • Prevalentie 40-50% (20% voor motorische symptomen) • vaak milde symptomen • Verminderd initiatief en zelfwaardering • Weinig suïcide, wel meer suïcidale ideaties • Vaak paniekaanvallen • Ernst niet in relatie tot motorische symptomen GeriatrieAvond Westfriesland
