Goal of the Family Support Centers • To serve as a “one-stop,” multi-service model of care for youth and their families • To divert youth who are FWSN referred with status offenses from further court involvement • To provide an array of services beginning with triage and concluding with successful placements with community-based services that meet the individual needs of the youth and families referred

  3. Family Support Center: Core Elements • Contact families within 3 hours of receiving the referral • Conduct initial screening to best determine the course of action for each family • Provide a comprehensive, strengths-based, gender- specific, relational and trauma-sensitive assessment • Utilize assessment information for appropriate planning of services offered within the center and making appropriate referrals with community service providers • Collaborate and coordinate with new and existing service providers to create a success plan for each family

Family Support Center:Staffing & Training • Interviews are on going and include a CSSD representative • Target date for staff: November 1, 2007 • Probation-scheduled Informational Sessions include FSC representatives • FSC collective meetings on-going • Training schedule • Fully operational as of December 1, 2007

Family Support Center:Current Referrals & Services • Waterbury FSC~ Connecticut Junior Republic • Hartford FSC~ Wheeler Clinic • Bridgeport FSC~ Connecticut Renaissance Inc. • New Haven FSC ~ St. Francis Home for Children, Inc. • Screening and intake process, crisis intervention, case coordination, access to probation-contracted services, CARE referrals, community services, DCF Voluntary Services and Systems of Care referrals

FWSN Informational Sessions • Connecticut Junior Republic, Waterbury: November 1st and November 7th 11:00a.m • Wheeler Clinic, Hartford: November 1st 1:00p.m. -5:00p.m. • Connecticut Renaissance, Bridgeport: October 23rd 2 2:30p.m. – 4:30p.m. and October 29th 3:0p.m. -6:00p.m. • St. Francis Home for Children, Inc., New Haven: October 23rd 3:00-5:00p.m. and October 30th 5:00-7:00p.m.
