Network Planning Program - Purpose • The purpose of this program is to promote the planning and development of healthcare networks in order to: • achieve efficiencies; • expand access to, coordinate, and improve the quality of essential health care services; and • strengthen the rural health care system as a whole. Applicants must describe planning activities that support at least one of the legislative aims above.

  9. Purpose Section Applicants must describe planning activities that support at least one of the legislative aims. Aim #1: Achieve efficiencies • The network will focus on identifying ways to achieve better system efficiencies and improve regional and/or local rural health care services. (See activities listed under this section, pages 2-3) Aim #2: Expand access to, coordinate, and improve the quality of essential health care services • The network will focus on ways to build capacity and a network infrastructure that enables entities to coordinate care and increase access to care for rural communities both locally and regionally. (Examples of planning activities within the legislative aim(s) #1, #2, and #3 , on page 34)

  10. Purpose Section (Continued) Applicants must describe planning activities that support at least one of the legislative aims. Aim #3: Strengthen the rural health care system as a whole • Network members will focus on ways to enhance community and partner relationships to promote involvement and participation in network planning activities. (See activities listed on page 3-4) Examples of promising practice, evidence-informed and/or evidence-based approaches available on the Rural Health Information Hub Website https://www.ruralhealthinfo.org/community-health/project-examples/evidence-levels * Applicants must clearly identify the legislative aim that your network planning activities support in the project abstract.

  11. Program Purpose– Focus Area Applicants should also clearly identify a focus area(s) that their network planning activities will address. The intent of the Network Planning program is to allow applicants the flexibility to determine their unique community needs and focus area(s), based on historical health care context, expertise, and relevant data sources in the community. * Applicants must clearly identify the focus area that your network planning activities support in the project abstract.

Eligibility Requirements – Ownership and Geographic Requirements • Nonprofit Status–must be nonprofit or public entity (p. 3 of NOFO) • Must Include proof of Nonprofit Status in Attachment 1 • Geography –The applicant organization must be a public or private non-profit entity located in a rural area or in a rural census tract of an urban county • Must meet rural according to HRSA’s definition (p. 4of NOFO) • All planned services must be provided in a rural county or census tract • Rural Health Grants Eligibility Analyzer: https://data.hrsa.gov/tools/rural-health?tab=Address • Partnership–must be in a network with at least 2 additional organizations (the lead applicant + at least 2 other orgs) (p. 1 of NOFO) • Project Requirements –must propose a project that is different from what was previously funded and have two new network members.

Eligibility Requirements - Network • Network must be composed of at least three members that are separate, existing entities, which have their own EIN number • Letter of Commitment (Attachment 9) and Network Organizational Chart and Network Member Information (Attachment 8) will be submitted with the application • Must have a Project Director in place, should be at least 25 % FTE • Must have at least 1.0 FTE of key staff on the grant (can include in-kind staff) • Applicants must clearly state who the project director is (and other key personnel on the award). Include name, title, and position on awardin the project abstract. • Please make sure the person listed on the face page (of the Application for Federal Assistance SF-424 form) as the point of contact/project director is who will be the project director

Eligibility Requirements – Funding History • If the applicant organization has a history of receiving funds under the Rural Health Network Development Planning award, they must propose a project that is different from what was previously funded and have two new network members. (p. 7 of NOFO) * Submit abstracts from previous Network Planning awards must be submitted in Attachment 11

  15. Eligibility Information: Frequently Asked Question #1 Q: My organization is located in a metropolitan area but we serve rural communities. Are we eligible to apply as the lead? A. No. Page 3 – “If the applicant organization’s headquarters are located in a metropolitan or urban county, that also serves or has branches in a non-metropolitan or rural county, the applicant organization is not eligible solely because of the rural areas they serve, and must meet all other eligibility requirements.”

  16. Eligibility Information: Frequently Asked Question #2 Q: We are a rural, public entity owned by an urban health care system. Are we eligible to apply as the lead applicant? A: The applicant organization may still apply as long as they have their own EIN (different from the urban or ‘parent’ organization). The rural site must be able to receive and administer the grant funds. (p. 4of NOFO). If applicable, the applicant must submit Attachment 10: Letter from Urban Parent Organization Consider working with a community organization that meets the eligibility criteria and be a part of the network/consortium involved in the project.

State Office of Rural Health Applicants are required to notify the State Office of Rural Health (SORH) of their intent to apply to this program A list of the SORHs can be accessed at https://nosorh.org/nosorh-members/nosorh-members-browse-by-state/. * Applicants must include in Attachment 2a copy of the letter or email sent to the SORH describing their project and any response to the letter received.

Funding Preferences Qualification 1: Health Professional Shortage Area (HPSA) http://datawarehouse.hrsa.gov/geoadvisor/ShortageDesignationAdvisor.aspx Qualification 2: Medically Underserved Community/Populations (MUC/MUPs) http://datawarehouse.hrsa.gov/geoadvisor/ShortageDesignationAdvisor.aspx Qualification 3: Focus on primary care and wellness and prevention strategies. Applicants applying for Qualification 3 must include a brief justification (no more than 3 sentences) describing how your project focuses on primary care and wellness and prevention strategies in Attachment 12. *Funding preference request must be included in the Project Abstract * Proof of meeting funding preference (screenshot or printout) and a statement of eligibility must be included in Attachment 12

Narrative Section – Introduction • Explain how your proposal incorporates elements of health care redesign, with a focus on transforming the health care delivery into a patient and value-driven system. This includes, but is not limited to, supporting the current health care landscape to improve outcomes, reduce costs, ensure access and efficient transitions of care, and promote innovative approaches. Please explicitly state the focus area(s) and legislative aim(s) that your program will address in this section. More information on how to address this is in the“Purpose” section.

Funding Restrictions • Based on historical data gathered from prior award cycles, equipment costs for this program have averaged 5 percent or less of the total award amount. • Based on historical data gathered from prior award cycles, legal costs for this program have averaged 20 percent or less of the total award amount. • Funds cannot be used to build or acquire real property or for construction • Funds cannot be used to pay for the direct provision of clinical health services.

Budget Information In addition to requirements included in the program-specific NOFO, include the following in the Budget Justification narrative: • Personnel Costs • Travel • Equipment • Supplies • Contractual • Other • Indirect Costs   Remember to also refer to the HRSA-SF-424 Application Guide as referenced throughout the NOFO

  25. How to Apply Where to Start? Grants.gov • Before any entity can work with the Federal Government through grants or contracts they must register in Grants.gov

How to Apply Required Steps for Grants.gov Registration • These Steps MUST be completed prior to registering for Grants.gov: • Obtain an organizational Data Universal Number System (DUNS) number • Register the organization with System for Award Management (SAM) • Register an Authorized Organization Representative (AOR)

  27. How to Apply Dun & Bradstreet Number (DUNS) • Your organization must have a DUNS Number. • If your organization does not have one, you will need to go to the Dun & Bradstreet website at http://fedgov.dnb.com/webform to obtain the number. • Same day registration

How to Apply System for Award Management (SAM) • Your organization must register with the System for Award Management (SAM) • Ensure that your organization is registered with the SAM at http://www.sam.gov • An authorizing official of your organization must register • Registration can take 3 Days – 2 Weeks • To keep your SAM active, be sure to renew at least once each year. If your registration expires, you cannot submit a grant application until it is renewed. – Renewal can take up to 5 Days

How to Apply Authorized Organization Representative (AOR) • Complete your AOR profile on Grants.gov and create your username and password at http://www.grants.gov/web/grants/applicants/organization-registration.html • You will need to use your organization’s DUNS Number and SAM Registration to complete this step. • Same day registration

How to Apply Grants.gov Registration • Allow 3-5 business days to register with Grants.gov after DUNS and SAM registration • Go to Get Registered with Grants.gov (http://www.grants.gov/web/grants/register.html) • When the registration is approved, the submitting AOR will receive a confirmation e-mail and will be able to submit applications Make sure your application is complete Watch your email • An applicant must receive at least two emails by the application deadline: • Confirm • Validate

Review Process • Applications are reviewed and conducted by outside review panel • Look at “Review Criteria” section to see how the “Narrative” sections will be reviewed • Programs are awarded based on scores • Review summaries and scores are sent out to all applicants • Acceptance and non-acceptance letters sent out prior to award date • Notice of Award sent out prior to July 1, 2019 start date

Helpful Tips Application Planning • Check registrations in DUNS, SAM, and grants.gov! • Read the entire funding opportunity and HRSA SF-424 application guide • Check eligibility! • Does your organization have the technical expertise, the personnel, and the financial capacity? • Are all stakeholders supportive? • Is your organization prepared to do what it takes?
