Acute sore throat • Etiology : mostly viral Group A beta hemolytic streptococcus (GABHS) may also cause acute sore throat • Symptoms: • Centor score: • History of fever (1) • Absence of cough (1) • Tender anterior cervical LN (1) • Tonsillar exudates (1)
Acute sore throat • Low Centor score (0-2) – probability of GABHS is low • Higher centor score (3-4) – chances of GABHS is high and antibiotic should be prescribed
Acute sore throat: Treatment • Analgesia – Paracetamol preferred over Ibuprofen • Plenty of fluids – warm?
Acute sore throat: Treatment • Antibiotics are indicated in case of – • High Centor score • No symptomatic improvement in 7 days • High risk group • Color – pale • Hydration – reduced skin turgor • Activity – reduced • Respiratory – RR>60, grunting • Others
Acute sore throat: Treatment What antibiotic? • First line: Amoxicillin / Penicillin • If allergic to Penicillin: Clarithromycin
