  8. Try in procedure 1)Temporary crown의 제거 ejector, chisel 이용 Backhaus forcep Straight chisel 길병원 치과센터 보철과 손미향

  9. 2)표면의 temporary cement를 마른 cotton pellet을 이용하여 깨끗이 제거, 필요하면 pumice 사용 3)마취하지 않은 상태에서 try in 4)지대치를 미지근한 물로 wash한 후 casting을 장착 -finger pressure만을 이용 ( Don’t bite) 5)Seating시 걸리는 경우 -proximal contact point가 풍융 하거나 crown 내면에 irregularity가 있는 경우가 多 길병원 치과센터 보철과 손미향

  10. Contact check 1) Contact 부위를 floss silk로 통과시켜 저항력이 세면 강도 강하고 약하면 강도 약함. 2) 인접치 contact point나 반대측 contact point의 강도가 비슷해야 한다. 길병원 치과센터 보철과 손미향

  11. 3)Contact이 너무 tight한 경우 -수복물이 적절히 위치되지 못하고, 불편감을 유발하며 환자가 치실을 사용하기 어려움. -그 부위가 shiny burnished surface로 보이므로 이 부분을 조금 삭제 4)Contact이 너무 loose한 경우 -음식물이 잘 끼게 되어 치은에 해로움 -contact point에 soldering하여 더 볼록하게 해줌. 길병원 치과센터 보철과 손미향

  12. 5)사용 기구 -red rubber wheel을 straight handpiece에 연결하여 사용 -너무 거친 재료의 경우 : polishing 후 open contact의 위험성 있음 ★기타의 방법 -교합지를 proximal부위에 위치 시킨 후 casting restoration을 place & remove. -casting restoration의 proximal 부위에 name pen과 같은 유성 pen을 칠하고 place & remove. 길병원 치과센터 보철과 손미향

  13. ★바람직한 contact point는… 1.Occluso-Gingival aspect -대부분의 구치들은 occlusal 1/3에 위치. (just below marginal ridge) -상악 제 1 대구치과 상악 제 2대구치 사이는 middle 1/3에 위치. -contact은 point보다 커야 하나 gingival embrasure를 침범해서는 안됨. -size: 1~2mm 길병원 치과센터 보철과 손미향

  15. 2.Facio-lingual aspect -contact point가 faciolingual로 가운데 위치하는 상악 제 1대구치와 제 2대구치 사이를 제외하고는 구치들의 contact point는 middle보다 약간 facial 쪽에 위치한다. -결과적으로 lingual embrasure는 facial embrasure보다 커서 음식물이 혀쪽으로 빠져나가는 것을 용이하게 함. -size: 2~3mm 길병원 치과센터 보철과 손미향

  18. margin check 1) margin의 적합도 check  explorer를 직각으로 세워 긁어 본다. cf) sharp explorer tip의 직경은 80µm 2)어느 한 부위에 미세한 차이가 있을 때 burnisher를 이용하여 매끄럽게 한다. 3)margin-subgingival margin일 경우는 구강 내에서 직접 안되므로 die 상에서 한다. 4)기름 묻힌 white polishing stone으로 회전방향이 주조체의 margin에서 치질 쪽으로 low speed로 margin을 polishing 5)Try in 과정에서 margin부위가 망가지기 쉬우므로 polishing 끝난 후 구강 내에서 제거할 때 margin 부위가 상하지 않도록 주의한다. 길병원 치과센터 보철과 손미향

Occlusal adjustment 1)Centric adjustment -Articulating paper (carbon paper)을 이용하여 i) premature를 제거 ii)꽉 물릴 때 하악이 미끌리지 않게 조절 <Rule> ①centric에서 cusp닿고, eccentric에서 cusp 닿지 않으면 fossa deepening ②centric과 ecentric에서 cusp가 닿으면 cusp을 grinding-true marking 찾아서 삭제해야 한다. ③shim stock(5/10000 inch =13µm 셀로판지)이용 길병원 치과센터 보철과 손미향

  26. contours • 부적절한 contour는 치은의 건강을 해치며 정상적 형태를 이루지 못한다. • 영구 접착전에 수정할 것. • 치은연 근처에서 과도하게 볼록하면 치태의 축적을 용이하게 한다. 교합면에서 치근 분지부쪽으로 가는 면은 보통 오목한데, 수복물의 축벽에서는 교합면까지 오목하게 만들어 주어야 치솔이 도달하기 쉽다(emergence profile고려). 길병원 치과센터 보철과 손미향

  28. Esthetics • 한걸음 물러서서 대화를 할 만한 거리에서 수복물을 보았을 때 수복물의 형태가 환자의 다른 치열과 조화를 이루는지 확인한다. 환자에서 거울을 보게 하여 수복물을 접착하기 전에 외형에 대한 불만이 없는지 확인하여 조정한다. 길병원 치과센터 보철과 손미향

  29. burnishing & polishing 1.Burnishing -정의:gold를 grinding, spinning, drawing, pulling 하는 것 -목적: 좀더 정확하게 적합 ( metal는 30%이상 elongation이 가능, 20% may be OK) -사용기구:white stones, petrolatum or round steel bur, finishing bur green stone은 사용하지 말것(abrade too much) -final smoothing: 3/8 inch cuttle disc and petrolatum 길병원 치과센터 보철과 손미향

  33. 금관에 비해 취급하기가 더 어렵고 환자가 삼키기 쉬움 시적하고 접착하는 과정은 rubber dam을 장착하고 시행. • 구강 내에서 cone-shaped white stone을 이용하여 강한 압력을 가하면서 자연치아의 열구를 금속에 연장시킴으로써 교합면 형태와 변연을 동시에 재조정. 길병원 치과센터 보철과 손미향

  34. Explorer로 와동에서 빼냄. 이때 잘 빠지지 않으면 압축공기를 불어본다. 이것도 실패하면 Richwil crown remover를 이용. • Polishing은 die상에서 시행 • 접착 전에 와동을 vanish 또는 dentin bonding agent로 코팅한 다음 와동에 시멘트를 채우고 inlay를 삽입. • 시멘트가 경화된 후 rubber dam을 제거 하고 교합조정과 polishing시행. 이때 치아를 과열하지 않도록 주의. 길병원 치과센터 보철과 손미향
