  6. EOP Networking Form Form # 5 Networking Form 1. What other question could be added to or subtracted from the Executive Order 665 Survey (Form #1)? What issues specific to your campus would make this form useful, or ineffective? 2. What other questions or comments may be added to or subtracted from the probation checklist (Form #2)? What issues specific to your campus would make this form useful, or ineffective? 3. What other questions or comments may be added to or subtracted from the Conditions for readmission form (Form #3)? What issues specific to your campus would make this form useful, or ineffective? 4. Is there anything we could add or subtract from the exit interview form (Form #4) to make it more useful? What issues specific to your campus would make this form useful, or ineffective? We are currently experimenting with text messaging students. Can you think of other creative ways, besides calling, email, and sending post cards, to contact students?

  7. Recapture data from 2005 • Our office started with 913 undecided first time freshman in the fall semester of 2005. • From that group we had 79 students disqualified in spring 2006, 24 disqualified in the fall of 2006, 26 disqualified in the spring of 2007, 21 disqualified in the fall of 2007. • A total of 150 students from the group of 913 undecided first time freshman were disqualified over the course of four years from fall 2005 to spring 2007. Success rate: • We were able to recapture 65 of the 150 students disqualified from the original group. Challenges include: • Locating students whose contact information has changed. • Making sure students meet the requirements for returning to the university before the university’s deadlines. • After students declare a major, following their matriculation to see if they make it to graduation
